The Wood County Health Department recently conducted inspections at food-service establishments in Rossford, Bowling Green, Northwood and Perrysburg.

The following inspections were done Jan. 23.

Ameristop Food Mart #29604, 395 E. Andrus Road, Northwood, had one critical and two non-critical offenses.

Critical was food not properly protected from contamination by separation, packaging, and segregation. It was corrected during inspection.

Non-critical were improper storage of food items (repeat); and non-food contact surfaces not easily cleanable.

During a follow-up inspection, Norplas Industries, 7825 Caple Blvd., Northwood, had one critical and one non-critical offense.

Critical was quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness.

Non-critical was facility does not have an employee with manager certification in food protection.

Perrysburg Cancer Center-Mercy Health, 12623 Eckel Junction Road, Perrysburg, had two critical offenses, which were quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness (corrected during inspection); and food contact surfaces not easily cleanable.

The following inspections were done Jan. 24.

Cinemark USA, Inc., 2005 Hollenbeck Dr., Perrysburg, had one critical and one non-critical offense.

Critical was equipment and utensils contacting temperature-controlled food not cleaned every four hours.

Non-critical was outer opening not protected (repeat).

Slater Family Ice Arena, 417 N. Mercer Road, Bowling Green, had one critical offense, which was refrigerated, ready-to-eat, temperature-controlled foods not properly date marked. It was corrected during inspection.

Burger King #7195, 3812 Woodville Road, Northwood, had one critical and two non-critical offenses.

Critical was temperature-controlled foods not being hot held at the proper temperature. It was corrected during inspection.

Non-critical were food employee(s) not wearing a hair restraint (corrected during inspection); and non-food contact surfaces of equipment are unclean (repeat).

Fraternal Order of Eagles Aerie 2322, 658 Lime City Road, Rossford, had one critical and three non-critical offenses.

Critical was equipment food-contact surfaces or utensils are unclean. It was corrected during inspection.

Non-critical were presence of dead pests or insects (a dead mouse was observed on the floor of the dry storage room and another on the floor below the fryer); single-use utensils in the kitchen stored not all facing the same direction; and non-food contact surfaces of equipment are unclean (repeat).

YaYa Kitchen, 25950 N. Dixie Hwy., Perrysburg, had three critical and three non-critical violations.

Critical were handwashing sink blocked by roll-away cart (repeat, corrected during inspection); food not properly protected from contamination by separation, packaging, and segregation (repeat, corrected during inspection); and refrigerated, ready-to-eat, temperature-controlled foods not properly date marked.

Non-critical were improper method for cooling temperature-controlled foods (repeat, corrected during inspection); improper storage of food items (repeat); and non-food contact surfaces of equipment are unclean (corrected during inspection).

The following inspection was done Jan. 25.

During a follow-up inspection, Balance Pan-Asian Grille, 26520 N. Dixie Hwy., Perrysburg, had two critical and eight non-critical violations.

Critical were raw animal food (chicken) not cooked to required time and temperature (corrected during inspection); and ready-to-eat, temperature-controlled food not properly discarded when required.

Non-critical were no towels or drying device at the handwashing sink(s); label on a food packaged in-house did not contain all required information; improper storage of food items (corrected during inspection); food employee(s) not wearing a hair restraint; no sanitizer test kit available; accumulation of refuse, recyclables, or returnables; facility ceiling not maintained clean; and food service operation did not have a person in charge per shift with the person in charge certificatied in food protection.

The following inspections were done Jan. 26.

During a follow-up inspection, El Vaquero, 26611 N. Dixie Hwy., Perrysburg, had two critical and six non-critical violations.

Critical were chlorine sanitizing solution at incorrect temperature and/or concentration; and presence of live insects, rodents, and other pests (gnat-like pests were observed in kitchen and bar at time of inspection).

Non-critical were improper storage of food items (cases of syrup directly on floor in dry stock room); improper storage of food items (ice station observed with missing cover); equipment and utensils are not being air dried; non-food contact surfaces not easily cleanable; warewashing equipment and/or components not cleaned at required frequency; and physical facilities not maintained in good repair.

Courtyard Café, 9789 Clark Dr., Rossford, had one critical and four non-critical offenses.

Critical was ready-to-eat, temperature-controlled food not properly discarded when required (repeat, corrected during inspection).

Non-critical were improper storage of food items (repeat); equipment components are not intact, tight or properly adjusted (repeat); non-food contact surfaces of equipment are unclean; and physical facilities not maintained in good repair (repeat).

Hampton Inn, 142 Campbell Hill Road, Bowling Green, had one critical and one non-critical offense.

Critical was improper use of time as a public health control (four hours). It was corrected during inspection.

Non-critical was food employee(s) not wearing a hair restraint. It was corrected during inspection.

Target Stores T-1317, 9666 Old U.S. 20, Rossford, had one critical offense, which was improper storage of poisonous or toxic materials (repeat, corrected during inspection).

Susie’s Skillet, 101 S. Main St., Walbridge, had four critical and five non-critical violations.

Critical were employees are not informed in a verifiable manner of their responsibility to report information about their health; temperature-controlled foods not being hot held at the proper temperature; raw shell eggs not maintained at 45° F or below; and refrigerated, ready-to-eat, temperature-controlled foods not properly date marked. All were corrected during inspection.

Non-critical were latex gloves being used in the food service operation or retail food establishment (corrected during inspection); fixed equipment not properly sealed or spaced for cleaning; non-food contact surfaces of equipment are unclean; plumbing system not properly maintained or repaired; and facility walls not maintained clean.

Circle K #4705640, 103 N. Prospect St., Bowling Green, had two critical and five non-critical violations.

Critical were food not properly protected from contamination by separation, packaging, and segregation; and refrigerated, ready-to-eat, temperature-controlled foods not properly date marked.

Non-critical were no soap at handwashing sink(s); handwashing sink water below 100°F (repeat); no towels or drying device at the handwashing sink(s); non-food contact surfaces of equipment are unclean (repeat); and physical facilities not maintained in good repair.

Lupitas’ Cantina, 209 Louisiana Ave., Perrysburg, had one critical offense, which was food not properly protected from contamination by separation, packaging, and segregation. It was corrected during inspection.

The following inspections were done Jan. 27.

Northwest Community Corrections Center, 1740 E. Gypsy Lane Road, Bowling Green, had one critical and one non-critical offense.

Critical was quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness. It was corrected during inspection.

Non-critical was floors, walls, and/or ceilings not smooth and easily cleanable.

Wood County Haven Health Care, 1965 E. Gypsy Lane Road, Bowling Green, had one critical and one non-critical offense.

Critical was improper storage of poisonous or toxic materials. It was corrected during inspection.

Non-critical was non-food contact surfaces not easily cleanable (repeat).
