A Perrysburg ice creamery had four critical health code violations during a recent inspection, including
unclean utensils.
The Wood County Health Department inspected Twisty Treat on March 25.
The store, at 750 W. Boundary St., also had six non-critical violations.
Critical included handwashing sink is being used for purposes other than handwashing; equipment
food-contact surfaces or utensils are unclean; temperature-controlled foods not being hot held at the
proper temperature; and refrigerated, ready-to-eat, temperature-controlled foods not properly date
marked.
Non-critical included facility does not have an employee with level two certification in food protection
(repeat); improper storage of food items; in-use utensils improperly stored; re-use of single-service or
single-use articles; equipment not approved by a recognized testing agency; and toilet room not
completely enclosed and/or did not have tight-fitting, self-closing door.
The following inspections were done March 20.
Lee Williams Meats Inc., 941 Dixie Hwy., Rossford, had two non-critical violations during a follow-up
inspection: the non-food contact surfaces were dirty and not easily cleanable, and the inspector
observed holes in the ceiling in the back room and damaged and falling ceiling tiles throughout the
facility.
The following inspections were done March 21.
American Legion Post #539, 539 E. South St., North Baltimore, had one critical and three non-critical
violations.
The critical, corrected during inspection, was temperature-controlled foods not being cold held at the
proper temperature.
Non-critical were food not stored at least 6 inches off the floor to prevent contamination; single-use
containers re-used for food storage; and towels in use as lining throughout the facility.
Fuel Mart, 2185 Grant Road, North Baltimore, had two critical and four non-critical offenses.
Critical were facility did not have signed copies of the employee health policy on file, and the cleaning
frequency of equipment food contact surfaces and utensils not maintained. Both were corrected at the
time of inspections.
Non-critical included facility does not have an employee with level two certification in food protection;
facility did not have written procedures for vomiting or diarrheal events (corrected during inspection);
frequency of cleaning is inadequate; and facility not maintained in good repair.
North Baltimore Subway, 113 N. Main St., North Baltimore, had two non-critical violations which included
facility did not have proper sanitizer for their Norovirus clean-up procedure at time of inspection, and
food and dry storage areas did not have a light intensity of at least 10 foot candles to ensure employee
safety.
Children’s Discovery Center, 11090 Avenue Road, Perrysburg, had four critical and two non-critical
offenses.
Critical included employees are not informed in a verifiable manner of their responsibility to report
information about their health; chlorine sanitizing solution at incorrect temperature and/or
concentration; temperature-controlled foods not being hot held at the proper temperature; and improper
use of a poisonous or toxic substance.
Non-critical included no written procedures for vomiting or diarrheal events, and food thermometer not
readily accessible.
Crossroads Café, 127 S. Main St., North Baltimore, had one critical and four non-critical offenses.
The critical, which was corrected during inspection, was facility did not have signed copies of employee
health agreement on file.
Non-critical included food not stored at least 6 inches off the floor to prevent contamination (corrected
during inspection); towels observed drying under clean dishes (corrected during inspection); unnecessary
items and litter on premises; and repairs needed in walk-in cooler.
Ashland Bancroft Gas & Express Mart, 1133 Buck Road, Rossford, had two critical and five
non-critical offenses.
Critical included employees are not informed in a verifiable manner of their responsibility to report
information about their health, and handwashing sink not accessible.
Non-critical were no written procedures for vomiting or diarrheal events (corrected during inspection);
label on a food packaged in-house did not contain all required information; working food containers not
properly labeled; improper storage of food items; and no sanitizer test kit available.
Denny’s Time Out Tavern, 5808 Woodville Road, Northwood, had three non-critical violations, including
person-in-charge did not ensure that the facility has written procedures for employees to follow when
responding to vomiting or diarrheal events; non-food contact surfaces are not designed and constructed
to allow easy cleaning or maintenance; and the physical facilities are not being maintained in good
repair.
No offenses were found during inspections of Poppin’ George of Wood County 1, 27525 Luckey Road,
Walbridge; Poppin’ George of Wood County 2, 27525 Luckey Road, Walbridge; Six Fifths, 120 W. South
Boundary St., Perrysburg, during a consultation inspection; Rosie’s Fine Foods JR, 28485 E. River Road,
Perrysburg; Wood County Rest Area operated by J. R.’s Vending Co. Inc, 12729 Interstate 75 Southbound,
Portage; Rite Aid, 1175 Louisiana Ave., Perrysburg; L.P. Stotz Farms, 8771 Albain Road, Ida; and Rite
Aid, 801 Dixie Hwy., Rossford;
Lee Garden, 26597 N. Dixie Hwy., Suite 161, Perrysburg, had zero violations after a complaint inspection.

A caller stated eating at facility on March 13 just after 6 p.m. and hearing a small child in kitchen
coughing and sneezing.
Inspectors spoke to person in charge and explained how children were not allowed in the kitchen at all.
Person in charge did admit his child was not feeling well and they kept him in the back storage area but
stated his children will not be present in the facility as they will have childcare available moving
forward.
McDonald’s, 2700 Woodville Road, Northwood, had zero violations during a complaint inspection.
The person in charge stated no employees have had long term illnesses in recent weeks. Past week of call
off logs observed and on March 19, an employee called off due to vomiting and did work on the morning of
March 22 when complainant got sick.
Person in charge stated hand washing is done every hour, chlorine sanitizer used and observed employees
on the line wearing gloves.
