Health department inspects restaurants

A Perrysburg restaurant had nine critical violations during a recent inspection.
The Wood County Health Department inspected Tokyo Express, LLC on Feb 7.
The restaurant, at 10013 Fremont Pike, also had seven non-critical violations.
Critical violations included: Handwashing sink is being used for purposes other than handwashing; food
not properly protected from contamination by separation, packaging, and segregation; food in contact
with an unclean surface or linens; food not properly protected from contamination by separation,
packaging, and segregation; food not properly protected from contamination by separation, packaging, and
segregation; equipment food-contact surfaces or utensils are unclean; refrigerated, ready-to-eat,
temperature-controlled foods not properly date marked; improper use of time as a public health control
(4 hours); food contact surfaces not easily cleanable.
Non-critical offenses included: Bulk food for customer self-service not properly labeled (corrected
during inspection); in-use utensils improperly stored; linens were not kept clean; re-use of
single-service or single-use articles; equipment not approved by a recognized testing agency (repeat);
non-food contact surfaces of equipment are unclean; mops dried improperly.
Chili’s Grill & Bar, 9886 Old U.S. 20, Rossford, had three critical and four non-critical
violations on Feb. 7.
All three critical offenses were corrected during inspection and included bar employee touching fruit
with bare hand to place it on the edge of a drink; build-up on the pop nozzles in the kitchen; and
time/temperature controlled for safety food – cold holding (repeat).
Non-critical violations included clean equipment and utensils not stored in a self-draining position and
covered or inverted; damage to the gasket of the batter/breading cooler (repeat); cleanliness of
nonfood-contact surfaces of equipment; repair (observed missing coving, multiple areas of low grout and
numerous ceiling tiles missing throughout the facility (repeat).
Waffle House, 26046 N. Dixie Hwy., Perrysburg, had four critical and three non-critical offenses during a
Feb. 7 inspection.
All four critical were corrected during inspection, and included chlorine sanitizing solution at
incorrect temperature and/or concentration; hot water for mechanical sanitization is below required
temperature; food equipment surfaces not cleaned at required frequency; and temperature-controlled foods
not being cold held at the proper temperature
Non-critical violations included improper use and/or maintenance of wiping cloths (a repeat offense
corrected during inspection); unclean nonfood-contact surfaces; and physical facilities not maintained
in good repair (repeat).
On Feb. 7, Slow Ride Saloon, 229 Main St., Luckey, had one critical and five non-critical offenses.
Refrigerated, ready-to-eat, temperature-controlled foods not properly date marked was a critical repeat
offense.
Non-critical included cooking and/or baking equipment not cleaned when required; improper use and/or
maintenance of wiping cloths; nonfood-contact surfaces not easily cleanable; non-food contact surfaces
of equipment are unclean; floors, walls, and/or ceilings not smooth and easily cleanable.
Heartland of Perrysburg, 10540 Fremont Pike, had two critical and two non-critical violations on Feb. 6.

Critical included handwashing sink is being used for purposes other than handwashing; and
temperature-controlled foods not being hot held at the proper temperature.
Both non-critical violations were corrected during inspection: employee food and drink in non-designated
area; and nonfood-contact surfaces not easily cleanable.
Royal Bar, 3012 Woodville Road, Northwood, had one critical and two non-critical violations on Feb. 6
Corrected during inspection was the critical violation that all items in a spray bottle or container must
have the common name to identify the contents.
Non-critical included unclean nonfood-contact surfaces, and floors, walls, and/or ceilings not smooth and
easily cleanable.
Burger King, 3812 Woodville Road, Northwood, had one critical and one non-critical on Feb. 6.
Corrected during inspection was the critical violation that temperature-control foods not being cold held
at the proper temperature.
A repeat non-critical was facility not maintained clean.
Eastern Lanes, 3511 Woodville Road, Northwood, had one non-critical that was corrected on inspection on
Feb. 6. In-use utensils were improperly stored.
Ameristop Food Mart, 395 E. Andrus Road, Northwood, had two non-critical violations on Feb. 6.
Nonfood-contact surfaces not clean, and improper drying of mops.
The Country Farmhouse, 117 E. Main St., Wayne, had one non-critical on Feb. 6 that was a repeat
violation. Outdoor refuse areas were not properly constructed.
Subway, 3310 Woodville Road, Northwood, had one critical and one non-critical violation on Feb. 6.
The critical, corrected during inspection, was working containers of poisonous or toxic materials not
properly labeled.
The non-critical was light intensity less than 10 foot candles in required areas.
On Feb. 6, Cocina de Carlos, 27072 Carronade Drive, Perrysburg, had three critical and two non-critical
violations.
Critical included handwashing sink is being used for purposes other than handwashing (corrected during
inspection); food not properly protected from contamination by separation, packaging, and segregation;
equipment food-contact surfaces or utensils are unclean.
Non-critical included re-use of single-service or single-use articles, and non-food contact surfaces of
equipment are unclean.
Perrysburg Senior High School, 13385 Roachton Road, had two non-critical violations on Feb. 6: Equipment
needs to be in good repair and proper adjustment; and observed cracked floor tiles near the floor drains
(repeat offense).
Marco’s Pizza, 629 W. South Boundary St., Perrysburg, had one critical and six non-critical offenses on
Feb. 7.
Improper storage of poisonous or toxic materials was the critical violation.
Non-critical included facility does not have a person with level two certification in food protection
(repeat); no soap at handwashing sink(s) (corrected during inspection); no sanitizer test kit available;
service sink or curbed cleaning facility not provided and/or conveniently located; cleaning/clean and
maintain the walk-in cooler fan guards; physical facilities not maintained in good repair.
Perrysburg Junior High School, 550 E. South Boundary St., had one non-critical violation on Feb. 7:
Handwashing sink – required water temperature (front handwashing sink water 88°F).
Perrysburg Commons, 10542 Fremont Pike, had one critical and three non-critical violations on Feb. 7.
Unclean equipment food-contact surfaces or utensils was the critical offense.
Non-critical included improper storage of food items; improper storage of single-service and single-use
articles; and non-food contact surfaces of equipment are unclean.
On Feb. 7, Frisch’s Big Boy, 10705 Fremont Pike, Perrysburg, had two critical and one non-critical
violation during a complaint inspection.
The complaint was from gentleman who ate at facility with wife on Feb. 6. He stated wife’s food was warm
on top and cold on the bottom and she was sick on Feb. 7.
Inspectors spoke to the person in charge. Call-off logs are attached to each employee’s health reporting
agreement and no call-offs were made within recent days. All food is fully cooked and person in charge
does have Level 2 food safety certification and answered food safety questions correctly.
The critical violations included food employee(s) did not wash hands when required; and
temperature-controlled foods not being cold held at the proper temperature.
The non-critical violation was non-food contact surfaces of equipment are unclean.
Chick-fil-A Restaurant, 10315 Fremont Pike, Perrysburg, had one critical and one non-critical offense on
Feb. 7.
Critical was equipment food-contact surfaces or utensils were unclean.
The non-critical was non-food contact surfaces of equipment were unclean.
On Feb. 7, Principle Business Enterprises, 20189 Pine Lake Road, Dunbridge, had one critical and one
non-critical violation.
The critical was quaternary ammonium sanitizing solution was at incorrect temperature, concentration,
and/or water hardness (corrected during inspection).
The non-critical was dressing rooms or lockers were not being properly used.
Stroh – The Perch, at 1535 E. Wooster St., Bowling Green, was inspected on Feb. 9 and had two
non-critical violations. In-use utensils improperly stored (corrected during inspection);
nonfood-contact surfaces not easily cleanable.
Stroh – Wooster St. Station, 1535 E. Wooster St., Bowling Green, was inspected Feb. 9 and had two
non-critical violations: Nonfood-contact surfaces not easily cleanable, and all personal items must be
stored below and away from facility food items,
Stroh – Bird Feeder, 1535 E. Wooster St., Bowling Green, was inspected Feb. 9 and had two non-critical
offenses: Nonfood-contact surfaces not easily cleanable (repeat); and employees items located above
facility items.
Restaurants that had zero violations during inspections held Feb. 6-9 included Hull Prairie Intermediate
School, 25480 Hull Prairie Road, Perrysburg; Speedway, 3402 Woodville Road, Northwood; Avina Auction,
142 E. Strong Ave., Wayne; Fort Meigs Elementary School, 26431 Fort Meigs Road, Perrysburg; and Stroh –
Anderson Club, 1535 E. Wooster St., Bowling Green.
