Theodore "Smoke" R. Slomkowski, of Rudolph, Ohio, passed away on August 18, 2011.
He is survived by his life partner of 42 years, Mildred Guy; his stepchildren, Tony Guy, Christine (Guy)
and Alvin Kilburn, Jack Guy, and children, Pam (Slomkowski) and Dan Malcom, Krys (Slomkowski) Bart and
husband, Mike Liccardello, Diane Slomkowski. He was preceded in death by his son, Ron Slomkowski,
stepdaughter, Rose Clagg and grandson, Sherman Clagg.
He also is survived by his grandchildren, Venessa and Jason Guy, Nicole Guy and Dennis Kilburn, Jessica,
Jackie, Jack Jr., Jonathon, Jacob and Joseph Guy, Jack Phillip Simon, Adam, Heath and Christopher
Malcom, John and Jeffery Pusher and Jay Bart, Lori Betz, Debbie Snyder and Ron Slomkowski and 22
great-grandchildren.
Smoke was an expert rifleman and an airplane mechanic and became a corporal in the 4th division of the
United States Marine Corps. He participated in WWII in Iwo Jima, he had an avid love for fishing and
hunting and could still split an arrow at 90 years old.
He was a certified electrician and refrigeration engineer with ammonia; worked as a maintenance engineer
for Bonds Bakery and Williams Provision.
Smoke was a man of few words but full of intelligence and life lessons.
The Memorial Mass with Remains will be held on Aug. 27, 2011 at 2 p.m. at Holy Family Catholic Church,
located at 115 E. Cherry St., North Baltimore, OH 45872. Family and friends are invited to participate.

Please send no flowers, make donations to Bridge Hospice Care Center of Bowling Green in the name of
Theodore Slomkowski for all of the excellent care that they have provided.
There will be a light dinner at the residence for family and friends that follows.
