Wood County Health Department conducts inspections

The Wood County Health Department started the new year inspecting food establishments in Bowling Green and Perrysburg.

The following inspections were done Jan. 5.

Sunset Bistro, 1220 W. Wooster St., Bowling Green, had one critical and one non-critical offense.

Critical was improper reheating of food for hot holding (repeat, corrected during inspection).

Non-critical was employee eating, drinking, or using tobacco in non-designated area.

Brookdale – Bowling Green, 121 N. Wintergarden Road, had one non-critical offense, which was physical facilities not maintained in good repair.

Clare Bridge of Bowling Green, 121 N. Wintergarden Road, had one critical and one non-critical offense.

Critical was chlorine sanitizing solution at incorrect temperature and/or concentration (repeat, corrected during inspection).

Non-critical was physical facilities not maintained in good repair.

The Sweet Stop, 1220 W. Wooster St., Bowling Green, had five non-critical offenses, which were facility does not have an employee with manager certification in food protection (repeat); no soap at handwashing sink(s); outer opening not protected; physical facilities not maintained in good repair; and ventilation hood system not sufficient to prevent grease or condensation build-up (repeat).

Clean Juice, 3155 Levis Commons, Suite 260, Perrysburg, had one critical offense, which was temperature-controlled foods not being cold held at the proper temperature.

The following inspections were done Jan. 6.

During a follow-up inspection, Petro Fuel Island, 26416 W. Baker Drive, Perrysburg, had one critical and one non-critical offense.

Critical was water source and/or hot water capacity not sufficient.

Non-critical was plumbing system not properly maintained or repaired.

Local Roots Juice Co., 306 Elm St., Perrysburg, had five non-critical violations, which were improper storage of food items (repeat); improper storage of cleaned equipment, utensils, and laundered linens; fixed equipment not properly sealed or spaced for cleaning; non-food contact surfaces not easily cleanable; and non-food contact surfaces of equipment are unclean (repeat).

Waffle House, 11121 Fremont Pike, Perrysburg, had two critical and four non-critical violations.

Critical were handwashing sink is being used for purposes other than handwashing; and temperature-controlled foods not being cold held at the proper temperature (repeat, corrected during inspection).

Non-critical were improper method for cooling temperature-controlled foods; food employees wearing jewelry on arms or hands during food preparation; equipment and utensils are not being air dried; and non-food contact surfaces of equipment are unclean (repeat).

True Spice, 13003 Eckel Junction Road, Perrysburg, had one non-critical offense, which was unnecessary or nonfunctional items and /or litter on premises (repeat).

The following inspection was done Jan. 9.

During a follow-up inspection, Walmart, 131 W. Gypsy Lane Road, had four non-critical offenses, which were food was unsafe, adulterated, or not honestly presented; edible printing items including edible paper stored in cabinet under prep sink in bakery; cooking equipment or pan surfaces unclean; and non-food contact surfaces of equipment are unclean.

The following inspections were done Jan. 10.

Best Western Falcon Plaza, 1450 E. Wooster St., Bowling Green, had three critical offenses, which were temperature-controlled foods not being cold held at the proper temperature; improper use of time as a public health control (four hours) (repeat); and improper storage of poisonous or toxic materials (corrected during inspection).

During a follow-up inspection, Grape Leaf Express, 27112 Oakmead Drive, Perrysburg, had five non-critical violations, which were improper storage of food items; food employee(s) not wearing a hair restraint; improper storage of single-service and single-use articles; cooking equipment or pan surfaces unclean; and non-food contact surfaces of equipment are unclean

S&G, 28311 Parkway Blvd., Rossford, had one critical and two non-critical offenses.

Critical was equipment food-contact surfaces or utensils are unclean. It was corrected during inspection.

Non-critical were non-food contact surfaces of equipment are unclean (repeat); and floors, walls, and/or ceilings not smooth and easily cleanable.

Moe’s Place, 620 Dixie Hwy., Rossford, had three critical offenses, which were food not properly protected from contamination by separation, packaging, and segregation; quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness (repeat); and refrigerated, ready-to-eat, temperature-controlled foods not properly date marked (repeat). All were corrected during inspection.

Subway, 131 W. Gypsy Lane Road, Bowling Green, had one critical and one non-critical offense.

Critical were temperature-controlled foods not being cold held at the proper temperature (repeat).

Non-critical was working food containers not properly labeled. It was corrected during inspection.

Grounds For Thought, 174 S. Main St., Bowling Green, had one critical and one non-critical offense.

Critical was quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness. It was corrected during inspection.

Non-critical was non-food contact surfaces not easily cleanable (repeat).

McDonald’s, 1050 S. Main St., Bowling Green, had one non-critical offense, which was non-food contact surfaces of equipment are unclean (repeat).

Vito’s Pizza, 118 W. South Boundary St., Perrysburg, had three critical and eight non-critical violations.

Critical was temperature-controlled foods not being cold held at the proper temperature (corrected during inspection); refrigerated, ready-to-eat, temperature-controlled foods not properly date marked (corrected during inspection); and no air gap or approved backflow prevention device on the plumbing system.

Non-critical were cutting or piercing parts of can opener not maintained; non-food contact surfaces of equipment are unclean (repeat); cooking equipment or pan surfaces unclean; physical facilities not maintained in good repair; facility not maintained clean (repeat); light intensity less than 10 foot candles in required areas; light intensity less than 50 foot candles in required areas; and retain food establishment does not comply with Level One certification requirements.

Walmart, 10392 Fremont Pike, Perrysburg, had one critical and six non-critical violations.

Critical was food in a retain food establishment must be embargoed due to adulteration, misbranding, or baby food or formula is expired. It was corrected during inspection.

Non-critical were most recent water quality sample report for private water system not on file (repeat); damaged, spoiled, and/or recalled products not segregated from food, equipment, utensils, linens, and single-service and single-use articles (repeat, corrected during inspection); food employee(s) not wearing a hair restraint (corrected during inspection); equipment components are not intact, tight or properly adjusted; non-food contact surfaces of equipment are unclean (repeat); and physical facilities not maintained in good repair (repeat).

Rapid Fired Pizza, 852 S. Main St., Bowling Green, had one non-critical offense, which was non-food contact surfaces of equipment are unclean.

Chili’s Grill & Bar, 9886 Old U.S. 20, Rossford, had one critical and three non-critical offenses.

Critical was presence of live insects (live gnats were observed throughout bar area).

Non-critical were equipment and utensils are not being air dried; non-food contact surfaces of equipment are unclean (repeat); and facility not maintained clean (repeat).

Zero violations were found at Culver’s of Perrysburg, 10380 Fremont Pike; Benchmark Restaurant Group LLC, 6130 Levis Commons Blvd., Perrysburg; Primrose School of Perrysburg, 7123 Lighthouse Way; B Family Sushi and Asian Cuisine, 1234 N. Main St., Bowling Green; and Taco Bell, 27171 Oakmead Drive, Perrysburg.
