INVITATION TO
BID

SEALED BIDS for the furnishing of the necessary materials and construction of the
CARTER & WESTERN ELEVATED STORAGE TOWER
IMPROVEMENTS
will be received by the City of Bowling Green, Ohio at the office of the Director of Utilities, in the
City Administrative Services Building, 304 North Church Street, until
2:00 P.M. (Local Time)
AUGUST 7, 2009
and at that time and place will be publicly opened and read aloud.
The proposed work consists of:
CARTER
TOWER:
Spot cleaning and spot coating the base plate and areas of coating
damaged during modifications. Work items include: modification of access tube-to-roof detail, walkway
toe bar, access tube ladder, interior container ladder, interior wet platform handrail, interior wet
platform manhole, and flanged and bolted roof manhole; installation of new clog-resistant roof vent,
globe for light fixture, lower pedestal ladder safe-climbing device extension, and curb and cover for
access tube manhole to interior wet area; relocation of cables, conduits, and light fixtures; including
other miscellaneous repairs and incidental items such as coordination with the OWNER, first anniversary
evaluation, disposal of debris, site restoration, etc.
WESTERN
TOWER:
Spot cleaning and spot coating the base plate and areas of coating
damaged during modifications. Work items include: modification of pedestal ladders, access tube ladder,
walkway manhole, interior wet platform manhole, and flanged and bolted roof manhole; installation of new
bowl manhole ladder, walkway safety railing, painter’s manhole platform safety railing, and interior wet
platform safety railing; relocation of cables and conduits; including other miscellaneous repairs and
incidental items such as coordination with the OWNER, first anniversary evaluation, disposal of debris,
site restoration, etc.
The Bidders attention is directed to the Detailed Technical Specifications for a complete description of
the Work and the Project.
The contract documents, including drawings and specifications are on file at the office of the Utilities
Director, City of Bowling Green, 304 North Church Street, Bowling Green, Ohio 43402 and in the office of
the Engineer – Tank Industry Consultants – Headquarters: 7740 West New York Street, Indianapolis,
Indiana 46214-2988, telephone: (317) 271-3100, fax: (317) 271-3300 until the bid opening.
The contract documents, plans, specifications, and evaluation report may be reviewed at the above
locations by appointment only. Complete sets of the Bidding Documents for the project may be obtained
from Tank Industry Consultants by United States Priority Mail Service, for a non-refundable sum of forty
dollars ($40) each. Next day delivery service by United States Express Mail Service, Federal Express or
UPS Next Day Mail will be available from Tank Industry Consultants for a non-refundable surcharge of
twenty dollars ($20) above the cost of the specifications. Checks for the bid documents shall be made
out to and delivered to Tank Industry Consultants, 7740 West New York Street, Indianapolis, Indiana
46214.
Color copies of the photographs contained in the evaluation reports may be obtained from Tank Industry
Consultants by United States Priority Mail Service, for a non-refundable sum of two hundred dollars
($200) per set. Next day delivery service by United States Express Mail Service, Federal Express or UPS
Next Day Mail will be available from Tank Industry Consultants for a non-refundable surcharge of twenty
dollars ($20) above the cost of the photographs. Checks for color copies of the photographs are to be
made out to and delivered to Tank Industry Consultants, 7740 West New York Street, Indianapolis, IN
46214 and may be combined with the checks for the bid documents.
All materials furnished and labor performed incident to and required for the proper and satisfactory
execution of the contracts shall be furnished in accordance with the requirements of the drawings and
specifications, and any addenda thereto, prepared by Tank Industry Consultants, 7740 West New York
Street, Indianapolis, IN 46214, Fax: 317-271-3300.
All bids shall be signed and submitted on the blanks which are bound in the specifications. Bid blanks
shall not be removed from the specifications. Bids shall state the unit prices on the blanks provided
therefor; shall be enclosed in a sealed envelope marked “CARTER & WESTERN TOWER IMPROVEMENTS”;
must contain the name of every interested person, firm, or corporation in the enclosed sealed bid; and
be accompanied by a properly executed Non-Collusion Affidavit and Bid Guaranty to the City of Bowling
Green as described in the contract documents.
Submitting a Bid on the forms that will be bound with the Contract Documents shall
acknowledge that the tank and site have been inspected by the BIDDER or that the right to do so has been
waived.

The bid guaranty may be of two forms:
A bid Guaranty and Contract Bond in the amount of 100% of the bid using the forms provided in the
specifications. (The amount of the bid does NOT have to appear in this form.)
A certified check, cashier’s check or letter of credit in favor of the City of Bowling Green in the
amount of 10% of the bid. If the contract is awarded, a Contract Bond will be required, which is a 100%
payment and performance bond.
The successful bidder will be required to submit an affidavit to the City Finance Director that he has no
delinquent personal property taxes charged against him on the general tax list of personal property in
Wood County, Ohio, at the time this Bid was submitted.
Bids received after the scheduled bid opening date and time, or not accompanied by a satisfactory bid
guaranty, will neither be read nor considered.
The City of Bowling Green, Ohio reserves the right to reject any and all bids and to waive any
irregularity in any bid and to determine the lowest and best bidder.
No bidder may withdraw his bid for a period of 60 days after the scheduled closing time for the receipt
of bids.
Attention of Bidders is particularly called to the requirements as to conditions of employment to be
observed and minimum wage rates to be paid under the contract.
The City reserves the right to reject any and all bids, to waive any irregularities in bidding, and to
determine the lowest and best bid.

NOTICE TO
BIDDERS

Sealed proposals will be received at the Office of the Board of County Commissioners, One Courthouse
Square, Bowling Green, Ohio 43402 until 10:00 a.m. on August 13, 2009, for furnishing all labor,
materials and equipment necessary for the 2009 Tracy Road Reconstruction and Widening at SR 795.
At said time and place, all bid proposals shall be publicly opened and read aloud.
Contract documents, bid sheets, plans and specifications may be obtained on or after July 30, 2009 from
the Wood County Engineer, One Courthouse Square, Bowling Green, Ohio 43402 at a cost of $5.00 or $10.00
if requested to mail.
The Notice to Bidders is posted on the internet and may be viewed on Wood County’s web page at:

All waste materials generated from this project must be taken to the Wood County Landfill for disposal.

Each bidder is required to furnish with its proposal, a Bid Guaranty and Contract Bond in accordance with
Section 153.54 of the Ohio Revised Code. Bid Security furnished in Bond form, shall be issued by a
Surety Company or Corporation licensed in the State of Ohio to provide said surety.
Each Proposal must contain the full name of the party or parties submitting the proposal and all persons
interested therein. Each bidder must submit evidence of experience on projects of similar size and
complexity by means of prequalification requirements.
All contractors and subcontractors involved with the project will, to the extent practicable, use Ohio
products, materials, services and labor in the implementation of their project. Additionally, contractor
compliance with the equal employment opportunity requirements of Ohio Administrative Code Chapter 123,
the Governor’s Executive Order of 1972, and Governor’s Executive Order 84-9 shall be required.
Bidders must comply with the prevailing wage rates on Public Improvements in Wood County as determined by
the Ohio Bureau of Employment Services, Wage and Hour Divisions, and per Oho Revised Code Section 4115.

In accordance with the Ohio Revised Code Section 2909.33, the successful bidder will be required to
certify that it does not provide material assistance to any organization on the United States department
of state terrorist exclusion list. The Terrorist Exclusion List may be found on the Ohio Homeland
Security Web site at:

Certification shall be made by completing a Declaration Regarding Material Assistance/Nonassistance To A
Terrorist Organization Form.
Ohio Revised Code Section 3517.13 I(3) and J(3) requires that no agency or department of this state or
any political subdivision shall enter into any contract for the purchase of goods costing more than five
hundred dollars or services costing more than five hundred dollars with a corporation, individual,
partnership or other unincorporated business, association, including, without limitation, a professional
association organized under Chapter 1785 of the Revised Code, estate, or trust unless the contract
includes a certification that the individuals named in Revised Code Sections 3517.13(I)(1) and (J)(1)
are in compliance with the aforementioned provisions. The bidder is required to complete the affidavit
provided. Failure to submit the required form with the proposal/bid package could deem the bidder’s
response to be non-responsive and disqualified from receiving further consideration.
Wood County may reject all bids per Ohio Revised Code 307.90 division (A). The rejection of such shall
not result in accrual of any right, claim, or cause of action by any unsuccessful bidder against Wood
County. The Board of County Commissioners reserves the right to waive any irregularities in the form of
the bid that do not affect or destroy competitive bidding. In no case will an award be made until all
necessary investigations are made as to the qualifications of the bidder to whom it is proposed to award
the contract.

NOTICE TO
VENDORS

Sealed proposals will be received in accordance with law until 1:00 pm on August 13, 2009 at Evergreen
Middle School, 14544 County Road 6, Metamora, Ohio 43540 for one or more 72 passenger conventional
school buses. The proposals will be publicly opened in the Evergreen Board Office Conference Room at
that time and read immediately thereafter by the Treasurer.
Specifications for the units to be purchased are on file and may be obtained from Ron Whitehead,
Transportation Coordinator, Evergreen Bus Garage, 14544 County Road 6, Metamora, Ohio 43540.
Each bid must be accompanied by an approved Surety Company Bid Bond or a certified check upon a solvent
bank, made payable to the Evergreen Board of Education in an amount equal to ten percent (10%) of the
bid tendered, as a guarantee that the bidder will perform his bid. Successful bidders will be required
to submit to the Treasurer a statement relating to delinquent personal property taxes as per Ohio
Revised Code 5719.042. All bids must state that the bus, prior to delivery, will comply with Ohio
Department of Education safety regulations.
