BG woman indicted for fraud

A Bowling Green woman has been indicted for illegally using assistance program benefits.

A Wood County grand jury on June 26 indicted Melissa Kukla, 45, for tampering with records and illegal use of supplemental nutrition assistance program benefits or WIC program benefits, both third-degree felonies; grand theft, Medicaid fraud and Medicaid eligibility fraud, all fourth-degree felonies; and illegal use of supplemental nutrition assistance program benefits or WIC program benefits, a fifth-degree felony.

From Aug. 1, 2017-Nov. 30, 2023, she allegedly falsified or destroyed information provided to Job and Family Services to receive benefits, obtained more than $7,500 through deception.

From Aug. 1, 2017-Feb. 29, 2020, she allegedly made false statements to obtain reimbursement from the Medicaid program as well as falsified an application for enrollment in Medicaid valued at more than $7,500.

From Aug. 1, 2019-May 31, 2022, she allegedly accepted or transferred as well as solicited or accepted benefit programs valued at more than $7,500.
