The Wood County Health Department inspected several dozen food service establishments during the last week of March.

The following inspection was conducted March 27.

Fraternal Order of Eagles Aerie 2633, 140 N. Main St., North Baltimore, had one non-critical offense, which was non-food contact surfaces of equipment are unclean (repeat).

The following inspections were conducted March 28.

Campus Quarters, 107 State St., Bowling Green, had two critical and one non-critical offense.

Critical were chlorine sanitizing solution at incorrect temperature and/or concentration (repeat); and no air gap between beverage and ice bin drain and floor drain (repeat).

Non-critical was equipment components are not intact, tight or properly adjusted (repeat).

Circle K #4705669, 996 S. Main St., Bowling Green, had five non-critical offenses, which were bulk food for customer self-service not properly labeled; food not protected from contamination (repeat); single-service articles or single-use articles stored in prohibited area (repeat); physical facilities not maintained in good repair (repeat); and facility not maintained clean.

Jets Pizza, 1216 N. Main St., Bowling Green, had three non-critical offenses, which were working food containers not properly labeled; fixed equipment not properly sealed or spaced for cleaning; and non-food contact surfaces of equipment are unclean

The following inspections were conducted March 30.

Children’s Discovery Center, 7033 Lighthouse Way, Perrysburg, had one critical offense, which was improper storage of poisonous or toxic materials. It was corrected during inspection.

Subway #50324, 28400 Cedar Park Blvd., Perrysburg, had six non-critical violations, which were outer opening not protected; food employees wearing jewelry on arms or hands during food preparation; clean equipment and utensils not stored in a self-draining position and covered or inverted; fixed equipment not properly sealed or spaced for cleaning; non-food contact surfaces of equipment are unclean; and dressing rooms or lockers not being properly used.

Starbucks #19772, 10680 Fremont Pike, Perrysburg, had one critical and six non-critical violations.

Critical was no air gap or approved backflow prevention device on the plumbing system (repeat).

Non-critical were handwashing sink water below 100°F; re-use of single-service or single-use articles; equipment components are not intact, tight or properly adjusted (torn gaskets on two door reach in cooler); equipment components are not intact, tight or properly adjusted (ice machine in back of house leaking); plumbing system not properly maintained or repaired; and facility not maintained clean.

BG’s Frosty Fare, LLC, 990 W. Poe Road Unit 14, Bowling Green, had one non-critical offense, which was food employee(s) not wearing a hair restraint.

Truenorth Store #517, 1000 Buck Road, Rossford, had four non-critical offenses, which were employee eating, drinking, or using tobacco in non-designated area; poisonous or toxic chemicals did not have manufacturers label; improper use and/or maintenance of wiping cloths; and mops dried improperly.

During a follow-up inspection, Mr. Spot’s, 206 N. Main St., Bowling Green, had one critical offence, which was equipment food-contact surfaces or utensils are unclean.

Bob Evans Restaurants, LLC. #116, 10770 Fremont Pike, Perrysburg, had two critical and 10 non-critical violations.

Critical were food employee(s) did not wash hands when required; and temperature-controlled foods not being cold held at the proper temperature (repeat).

Non-critical were employee eating, drinking, or using tobacco in non-designated area; no towels or drying device at the handwashing sink(s) (corrected during inspection); improper storage of food items (repeat); non-food contact surfaces not easily cleanable; warewashing equipment and/or components not cleaned at required frequency; Non-food contact surfaces of equipment are unclean (repeat); cooking equipment or pan surfaces unclean (repeat); physical facilities not maintained in good repair; facility not maintained clean (repeat); and dressing rooms or lockers not being properly used (corrected during inspection).

Ayo El Grande LLC DBA El Zarape, 1616 E. Wooster St. Unit #1, Bowling Green, had four critical and seven non-critical violations.

Critical were food not properly protected from contamination by separation, packaging, and segregation (repeat); equipment food-contact surfaces or utensils are unclean (repeat); temperature-controlled foods not being cold held at the proper temperature; and refrigerated, ready-to-eat, temperature-controlled foods not properly date marked. All were corrected during inspection.

Non-critical were working food containers not properly labeled (corrected during inspection); improper storage of food items (corrected during inspection); clean equipment and utensils not stored in a self-draining position and covered or inverted (corrected during inspection); equipment not approved by a recognized testing agency; non-food contact surfaces of equipment are unclean (repeat); physical facilities not maintained in good repair; and facility not maintained clean.

Pizza Pub 516, LLC, 516 E. Wooster St., Bowling Green, had one critical and three non-critical offenses.

Critical was food employee(s) did not wash hands when required (repeat. Corrected during inspection).

Non-critical were non-durable equipment observed (repeat); non-food contact surfaces of equipment are unclean; and physical facilities not maintained in good repair (repeat).

Tim Hortons, 1011 Buck Road, Rossford, had one critical and four non-critical offenses.

Critical was chlorine sanitizing solution at incorrect temperature and/or concentration. It was corrected during inspection.

Non-critical were employee eating, drinking, or using tobacco in non-designated area; warewashing machine not operated according to the manufacturer’s instructions; non-food contact surface(s) not cleaned at the required frequency; and facility not maintained clean (repeat).

Rossford Carryout, 904 Dixie Hwy., Rossford, had two non-critical offenses, which were hot and cold holding equipment thermometer was missing, located incorrectly, or not easily readable; and no covered receptacle in women’s restroom.

The following inspections were done March 31.

Woodbury Market, 1503 Woodville Road, Millbury, had one critical and one non-critical offense.

Critical was food not properly protected from contamination by separation, packaging, and segregation. It was corrected during inspection.

Non-critical was physical facilities not maintained in good repair (repeat).

China City, 129 N. Main St., North Baltimore, had six critical and 10 non-critical violations.

Critical were handwashing sink not accessible; food not properly protected from contamination by separation, packaging, and segregation; equipment and utensils contacting temperature-controlled food not cleaned every four hours; temperature-controlled foods not being cold held at the proper temperature (repeat); refrigerated, ready-to-eat, temperature-controlled foods not properly date marked; and improper storage of poisonous or toxic materials. All were corrected during inspection.

Non-critical were employee eating, drinking, or using tobacco in non-designated area (corrected during inspection); unnecessary medicines and/or improper labeling/storing of medicines (corrected during inspection); non-durable equipment observed (repeat); non-food contact surfaces not easily cleanable (repeat); cooking equipment or pan surfaces unclean (repeat); non-food contact surfaces of equipment are unclean; facility not maintained clean (repeat); physical facilities not maintained in good repair (repeat); unnecessary or nonfunctional items and/or litter on premises (repeat); and light intensity less than 50 foot candles in required areas (repeat).

During a complaint inspection, Quality Inn, 10621 Fremont Pike, Perrysburg, had three critical and seven non-critical violations.

Inspectors spoke with the hotel manager regarding the complaint filed March 29 and learned the staff from the date of the complaint no longer worked at the facility.

Critical were unnecessary persons in the operation (corrected during inspection); employees are not informed in a verifiable manner of their responsibility to report information about their health; and improper use of time as a public health control (four hours) (repeat).

Non-critical were no written procedures for responding to vomiting or diarrheal events (corrected during inspection); no written procedures for time as a public health control (repeat); improper storage of single-service and single-use articles; latex gloves being used in the food service operation or retail food establishment (repeat, corrected during inspection); non-food contact surfaces not easily cleanable (repeat); unnecessary or nonfunctional items and/or litter on premises (repeat); and physical facilities not maintained in good repair.

The Village Pub, 1450 S Railroad St., Millbury, had four non-critical offenses, which were handwashing sink(s) are not conveniently located as specified in rule; no sanitizer test kit available; physical facilities not maintained in good repair; and food service operation did not have a person in charge per shift with the person in charge certification in food protection.

Zero violations were found at Dollar General Store #10840, 416 S. County Line Road, Risingsun; Kenwood Elementary, 710 Kenwood Ave., Bowling Green; Burger King #440, 1570 E. Wooster St., Bowling Green; Crim Elementary, 1020 Scott Hamilton Ave., Bowling Green; Lake Erie BBQ, LLC, 2461 Bedford Lane, Northwood; Knights Inn Toledo South, 1120 Buck Road, Rossford; St. Rose School, 217 E. Front St., Perrysburg; Circle K #4705640, 103 N. Prospect St., Bowling Green; Hillbilly Johnson’s Gourmet Dogs and More, 4041 Curtice Road, Northwood; and Falcon Eats, 705 Ridge St., Bowling Green.
