Health inspectors found mouse droppings on a table in a Bowling Green restaurant’s kitchen.

Wood County Health Department inspectors on Feb. 16 visited City Tap and the Attic, 110 N. Main St., and found two critical and eight non-critical violations.

Critical were food equipment surfaces not cleaned at required frequency (corrected during inspection); and presence of live insects, rodents, and other pests. The inspector observed multiple mouse droppings on the table in the kitchen.

Non-critical were facility does not have an employee with manager certification in food protection (repeat); handwashing sink(s) are not conveniently located; non-food contact surfaces not easily cleanable; utensils and/or equipment not properly rinsed (repeat, corrected during inspection); non-food contact surfaces of equipment are unclean (repeat); physical facilities not maintained in good repair (repeat); unnecessary or nonfunctional items and/or litter on premises; and facility not maintained clean (repeat).

The following inspections were done Feb. 13.

Pioneer Inn, 6751 Fremont Pike, Perrysburg, had one critical and eight non-critical offenses.

Critical was improper use of a poisonous or toxic substance. It was corrected during inspection.

Non-critical were food was unsafe, adulterated, or not honestly presented (corrected during inspection); unnecessary chemicals stored on premises; improper use or placement of insect control devices (corrected during inspection); improper storage of food items; non-durable equipment observed; non-food contact surfaces of equipment are unclean (repeat); unnecessary or nonfunctional items and/or litter on premises (repeat); and facility not maintained clean.

Domino’s Pizza, 154 E. South Boundary St., Perrysburg, had three non-critical offenses, which were fixed equipment not properly sealed or spaced for cleaning (repeat); equipment components are not intact, tight or properly adjusted; and non-food contact surfaces of equipment are unclean.

Kidz Watch, 580 Craig Drive, Perrysburg, had one critical offense, which was handwashing sink is being used for purposes other than handwashing.

During a follow-up inspection, Wings Over Bowling Green, 215 E. Wooster St., Bowling Green, had one non-critical offense, which was facility not maintained clean.

During a follow-up inspection, Chili’s Grill & Bar, 9886 Olde U.S. 20, Rossford, had three non-critical offenses, which were equipment and utensils are not being air dried; non-food contact surfaces of equipment are unclean; and facility not maintained clean.

5th Street Pub aka Hooper and Company LLC, 105-107 W. Fifth St., Perrysburg, had five critical and nine non-critical violations.

Critical were food in contact with an unclean surface or linens; quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness; temperature-controlled foods not being cold held at the proper temperature (repeat, corrected during inspection); refrigerated, ready-to-eat, temperature-controlled foods not properly date marked (corrected during inspection); and equipment and/or utensils improper construction (repeat).

Non-critical were employee eating, drinking, or using tobacco in non-designated area (repeat); inadequate equipment available for cooling, heating or holding food; food employee(s) not wearing a hair restraint (repeat); food employees wearing jewelry on arms or hands during food preparation; improper use and/or maintenance of wiping cloths; non-food contact surfaces not easily cleanable; non-food contact surfaces of equipment are unclean; physical facilities not maintained in good repair (repeat); and no protective shielding on lights

The following inspections were done Feb. 14.

Kermit’s Restaurant, 307 S. Main St., Bowling Green, had three critical and four non-critical violations.

Critical were chlorine sanitizing solution at incorrect temperature and/or concentration; ready-to-eat, temperature-controlled food not properly discarded when required; and improper storage of poisonous or toxic materials. All were corrected during inspection.

Non-critical were employee eating, drinking, or using tobacco in non-designated area; non-food contact surfaces not easily cleanable (repeat); non-food contact surfaces of equipment are unclean (repeat); and floors, walls, and/or ceilings not smooth and easily cleanable.

During a follow-up inspection, 5th Street Pub aka Hooper and Company LLC, 105-107 W. Fifth St., Perrysburg, had two critical and eight non-critical offenses.

Critical were food in contact with an unclean surface or linens; and equipment and/or utensils improper construction.

Non-critical were employee eating, drinking, or using tobacco in non-designated area; food employee(s) not wearing a hair restraint; food employees wearing jewelry on arms or hands during food preparation; improper use and/or maintenance of wiping cloths; non-food contact surfaces not easily cleanable; non-food contact surfaces of equipment are unclean; physical facilities not maintained in good repair; and no protective shielding on lights.

Belleville Market, 239 S. Main St., Bowling Green, had one critical and three non-critical offenses.

Critical was improper storage of poisonous or toxic materials (repeat). It was corrected during inspection.

Non-critical were label on a food packaged in-house did not contain all required information (repeat); non-food contact surfaces of equipment are unclean; and physical facilities not maintained in good repair (repeat).

During a complaint inspection, Bowling Green Dairy Queen, 434 E. Wooster St., had three non-critical offenses, which were handwashing sink water below 100°F; food employee(s) not wearing a hair restraint; and facility not maintained clean (repeat).

A complaint was received on Feb. 10 that the ketchup and mustard were at improper temperatures, no gloves or hair restraints were in use by food handlers, and the restroom was filthy.

The inspector observed debris on floors of both restrooms, ketchup bottles were kept at the tables (no refrigeration was needed), and only mustard packets were observed in facility.

Juniper Brewing Company, 145 S. Main St., Bowling Green, had four critical and four non-critical offenses.

Critical were temperature-controlled foods not being hot held at the proper temperature; temperature-controlled foods not being cold held at the proper temperature (repeat); refrigerated, ready-to-eat, temperature-controlled foods not properly date marked; and improper storage of poisonous or toxic materials. All were corrected during inspection.

Non-critical were working food containers not properly labeled (corrected during inspection); non-food contact surfaces of equipment are unclean; physical facilities not maintained in good repair (repeat); and employee personal items stored on shelf over food prep table.

During a follow-up inspection, S&G, 3510 Moline-Martin Road, Millbury, had two critical and four non-critical offenses.

Critical were sewage treated through an unapproved sewage treatment system; and sanitary waste was observed backing up into facility.

Non-critical were equipment components are not intact, tight or properly adjusted; plumbing system not properly maintained or repaired; service sink or curbed cleaning facility not provided and/or conveniently located; and facility not maintained clean.

The following inspection was done Feb. 15.

Meijer Gas Station, 2029 E. Wooster St., Bowling Green, had one critical and one non-critical offense.

Critical was refrigerated, ready-to-eat, temperature-controlled foods not properly date marked. It was corrected during inspection.

Non-critical was physical facilities not maintained in good repair (repeat).

The following inspections were done Feb. 16.

The China, 1039 Haskins Road, Bowling Green, had two non-critical offenses, which were non-food contact surfaces of equipment are unclean (repeat); and facility not maintained clean (repeat).

Cocina de Carlos, 27072 Carronade Drive, Perrysburg, had six critical and seven non-critical violations.

Critical were food not properly protected from contamination by separation, packaging, and segregation; chlorine sanitizing solution at incorrect temperature and/or concentration; equipment food-contact surfaces or utensils are unclean; temperature-controlled foods not being hot held at the proper temperature; temperature-controlled foods not being cold held at the proper temperature (repeat); and working containers of poisonous or toxic materials not properly labeled (repeat); All were corrected during inspection.

Non-critical were in-use utensils improperly stored (corrected during inspection); improper storage of single-service and single-use articles (repeat, corrected during inspection); cutting blocks or boards cannot be effectively cleaned and sanitized; fixed equipment not properly sealed or spaced for cleaning; non-food contact surfaces of equipment are unclean; cooking equipment or pan surfaces unclean (repeat); and facility not maintained clean.

Fat Fish Blue Home of the Funny Bone, 6140 Levis Commons Blvd., Perrysburg, had three critical and five non-critical offenses.

Critical were handwashing sink not accessible (repeat); ready-to-eat, temperature-controlled food not properly discarded when required; and improper storage of poisonous or toxic materials. All were corrected during inspection.

Non-critical were inadequate equipment available for cooling, heating or holding food (repeat); non-food contact surfaces of equipment are unclean throughout (repeat); physical facilities not maintained in good repair (repeat); facility not maintained clean; and no protective shielding on lights (repeat).

Uptown-Downtown, 162 N. Main St., Bowling Green, had eight non-critical violations, which were employee eating, drinking, or using tobacco in non-designated area; improper storage of single-service and single-use articles; non-food contact surfaces not easily cleanable (repeat); equipment components are not intact, tight or properly adjusted (repeat); utensils and/or equipment not properly rinsed; non-food contact surfaces of equipment are unclean; facility not maintained clean (repeat); and physical facilities not maintained in good repair (repeat).

Wood County Hospital Coffee Shop, 950 W. Wooster St., Bowling Green, had one non-critical offense, which was physical facilities not maintained in good repair.

Wood County Hospital, 950 W. Wooster St., Bowling Green, had two non-critical offenses, which were bulk food for customer self-service not properly labeled; and non-food contact surfaces not easily cleanable.

Neuroflex Juice Co., 650 Dixie Hwy., Rossford, had one non-critical offense, which was label on a food packaged in-house did not contain all required information.

During a follow-up inspection, S&G, 3510 Moline-Martin Road, Millbury, had one critical and three non-critical offenses.

Don’s Donut Shop, 2911 Woodville Road, Northwood, had three non-critical offenses, which were non-food contact surfaces of equipment are unclean; facility not maintained clean; and unnecessary or nonfunctional items and/or litter on premises All were repeat offenses.

Meijer Store, 2111 E. Wooster St., Bowling Green, had four non-critical offenses, which were non-food contact surfaces not easily cleanable; equipment components are not intact, tight or properly adjusted (repeat); physical facilities not maintained in good repair; and facility not maintained clean.

Bob Evans Restaurants LLC, 435 Lemoyne Road, Northwood, had one critical offense, which was temperature-controlled foods not being cold held at the proper temperature (repeat). It was corrected during inspection.

During a follow-up inspection, Staybridge Suites, 9749 Clark Drive, Rossford, had one critical and two non-critical offenses.

Critical was temperature-controlled foods not being cold held at the proper temperature.

Non-critical were facility does not have an employee with manager certification in food protection; and equipment components are not intact, tight or properly adjusted.

So Many Somethings Desserterie, 1021 Sandusky St., Perrysburg, had one non-critical offense, which was non-food contact surfaces of equipment are unclean.

Prime Thyme Eatery, 4505 Woodville Road, Northwood, had four non-critical offenses, which were cleaning agents and/or sanitizers not available in the food service operation or retail food establishment; non-food contact surfaces of equipment are unclean; facility not maintained clean; and mops dried improperly.

Hilton Garden Inn, 6165 Levis Commons Blvd., Perrysburg, had seven critical and five non-critical violations.

#317 Fork & Compass, 26416 W. Baker Drive, Perrysburg, had one critical and one non-critical offense.

Critical was temperature-controlled foods not being cold held at the proper temperature. It was corrected during inspection.

Non-critical was equipment and utensils are not being air dried.

Zero violations were found at SolFood Collective LLC, 119 W. Second St., Perrysburg; Country Catering, 1611 Pemberville Road, Northwood; South Side Six, 737 S. Main St., Bowling Green; and Graystone Banquet Hall and Conference Center, 29101 Hufford Road, Perrysburg.
