A Risingsun restaurant was inspected after a second complaint of a child in the facility’s work area was received.

The Wood County Health Department inspected Buddy’s Place, 100 Main St., on Sept. 23 after receiving a complaint the day prior that children were observed running around behind the store counters and near food being prepared.

The complaint was verified. A child was observed sitting behind the counter in the kitchen area at time of inspection. The inspector discussed with the person in charge that no unauthorized people are allowed in the food areas. The person in charge stated that they are working on setting up proper day care.

The inspector found one critical offense, which was unnecessary persons in the operation (repeat).

The establishment was inspected July 7 after the health department received a complaint that a small child was behind the deli counter where food was being prepared. A child was observed walking through the kitchen multiple times during that inspection.

The following inspections were done Sept. 19:

Circle K, 28350 Oregon Road, Perrysburg, had two non-critical offenses, which were no towels or drying device at the handwashing sink(s) (corrected during inspection); and non-food contact surface(s) not cleaned at the required frequency (repeat).

Belleville Market, 239 S. Main St., Bowling Green, had two critical and three non-critical violations.

Critical were refrigerated, ready-to-eat, temperature-controlled foods not properly date marked; and Improper storage of poisonous or toxic materials (repeat). Both were corrected during inspection.

Non-critical were unnecessary medicines and/or improper labeling/storing of medicines (corrected during inspection); label on a food packaged in-house did not contain all required information (repeat); and physical facilities not maintained in good repair (repeat).

Rudy’s Dairy Depot, 28572 Oregon Road, Perrysburg, had one critical and two non-critical offenses.

Critical was refrigerated, ready-to-eat, temperature-controlled foods not properly date marked. It was corrected during inspection.

Non-critical were non-food contact surfaces not easily cleanable (repeat); and non-food contact surfaces of equipment are unclean (repeat).

7820 Burger LLC, 7820 Ponderosa Road, Perrysburg, had one critical and two non-critical offenses.

Critical was working containers of poisonous or toxic materials not properly labeled. It was corrected during inspection.

Non-critical were non-food contact surface(s) not cleaned at the required frequency; and facility not maintained clean (repeat).

The following inspections were done Sept. 20.

Walt Churchill’s Market, 26625 N. Dixie Hwy., Perrysburg, had two critical and seven non-critical violations.

Critical were quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness (corrected during inspection); and bulk water machine was not sampled semi-annually and/or results not on site.

Non-critical were handwashing sink water below 100°F; improper storage of food items; food employees outer clothing was dirty; improper use and/or maintenance of wiping cloths; non-food contact surfaces of equipment are unclean (repeat); cooking equipment or pan surfaces unclean; and no protective shielding on lights (repeat).

1st Wok, 10093 Fremont Pike, Perrysburg, had two critical and five non-critical violations.

Critical were food not properly protected from contamination by separation, packaging, and segregation (corrected during inspection); and temperature-controlled foods not being cold held at the proper temperature.

Non-critical were improper method for cooling temperature-controlled foods (corrected during inspection); improper storage of food items (repeat); re-use of single-service or single-use articles (repeat); physical facilities not maintained in good repair (repeat); and unnecessary or nonfunctional items and/or litter on premises (repeat).

Fraternal Order of Eagles Aerie 2633, 140 N. Main St., North Baltimore, had one non-critical offense, which was non-food contact surfaces of equipment are unclean (repeat).

McDonald’s, 12776 Deshler Road, North Baltimore, had three non-critical offenses, which were food employee(s) not wearing a hair restraint; non-food contact surfaces of equipment are unclean (repeat); and facility not maintained clean (repeat).

American Legion Post 539, 539 E. South St., North Baltimore, had one critical and one non-critical offense.

Critical was refrigerated, ready-to-eat, temperature-controlled foods not properly date marked (corrected during inspection).

Non-critical was floors, walls, and/or ceilings not smooth and easily cleanable (repeat).

During a follow-up inspection, Tea Tree Asian Bistro, 4100 Chappel Drive, Perrysburg, had three critical and four non-critical violations.

Critical were written procedures and plans are not maintained and implemented; handwashing sink not accessible; and ready-to-eat, temperature-controlled food not properly discarded when required.

Non-critical were working food containers not properly labeled; improper use and/or maintenance of wiping cloths; improper storage of cleaned equipment, utensils, and laundered linens; and single-service or single-use articles, kitchenware, or tableware not handled, displayed, or dispensed properly.

Promedica Skilled Nursing and Rehabilitation (Perrysburg), 10540 Fremont Pike, had two critical offenses, which were temperature-controlled foods not being cold held at the proper temperature (repeat, corrected during inspection); and insufficient air gap between the flood rim and the water supply inlet.

The following inspections were done Sept. 21.

During a complaint inspection, Speedway 3643, 4661 Woodville Road, Northwood, had one non-critical offense, which was non-food contact surfaces of equipment are unclean.

The complaint was that a spill was behind the drink machines that had mold growth and an odor and that the workers had said the sewer backed up and it has not been repaired.

The inspector observed a dried red spill behind the slushie machine and in the cabinet below. The spills appeared to have build-up around them and there was also a slight odor present. Maintenance came prior to inspection and repaired the hose, but the cherry slushie is currently out of service. The inspector advised the person in charge that the spill shall be cleaned as soon as possible to prevent pests and to eliminate the odor.

Trotter’s Tavern, 119 N. Main St., Bowling Green, had three critical and four non-critical violations.

Critical were equipment food-contact surfaces or utensils are unclean (repeat); temperature-controlled foods not being cold held at the proper temperature (corrected during inspection); and improper use of time as a public health control (four hours) (repeat, corrected during inspection).

Non-critical were temperature-controlled foods not properly thawed (corrected during inspection); non-durable equipment observed; non-food contact surfaces not easily cleanable (repeat); and physical facilities not maintained in good repair.

During a follow-up inspection, Papa John’s Pizza 4714, 840 W. Boundary St., Perrysburg, had one critical and three non-critical offenses.

Critical was food package(s) received in poor condition.

Non-critical were food employee(s) not wearing a hair restraint; non-food contact surfaces of equipment are unclean; and retail food establishment does not comply with Level One Certification requirements.

During a complaint inspection, Ziggy Zoombas, 300 E. Wooster St., Bowling Green, had one non-critical complaint, which was the accumulation of refuse, recyclables, or returnables.

A complaint was lodged that the garbage bin was overflowing and there was garbage throughout the parking lot.

The inspector observed the complaint to be accurate. The person in charge said the trash was going to be picked up in the morning by the trash service. The trash service comes weekly but did not pick up last week due to items in front of the dumpster.

During a follow-up inspection, 2523 Oregon LLC, 2523 Oregon Road, Northwood, had one critical and four non-critical violations.

Critical was equipment food-contact surfaces or utensils are unclean.

Non-critical were equipment and utensils are not being air dried; no sanitizer test kit available; non-food contact surfaces of equipment are unclean; and physical facilities not maintained in good repair.
