A Perrysburg restaurant had a host of health code violations during a recent follow-up inspection,
including mixing raw meat with bare hands and not washing hands in an approved sink.
The Wood County Health Department inspected 1st Wok on March 27.
The store, at 10093 Fremont Pike, had four critical violations and five non-critical violations.
Critical included food employee(s) not washing hands in approved handwashing sink or automatic
handwashing facility (observed employee rinsing hands after touching raw shrimp in prep sink and not
using a designated hand sink); temperature-controlled foods not being cold held at the proper
temperature (repeat, egg rolls underneath prep top cooler at 46F and Kung Pao sauce at wok line at 63F);
temperature-controlled foods were not being held cold at the proper temperature (raw shell eggs not
maintained at 45 degrees or below); and food contact surfaces not easily cleanable (repeat).
Non-critical included excessive bare hand contact of raw foods (employee observed continuously mixing raw
meats with hands where arm was also touching food); food employee(s) hair not effectively restrained
(repeat, corrected during inspection); re-use of single-service or single-use articles (repeat);
non-durable equipment observed; and unnecessary or nonfunctional items and /or litter on premises.
The following inspections were conducted March 26.
Waffle House, 11121 Fremont Pike, Perrysburg, had two critical and three non-critical violations.
Critical included improper cooling of temperature-controlled foods, and temperature-controlled foods not
being cold held at the proper temperature.
Non-critical included facility does not have an employee with level two certification in food protection;
employee food and drink in non-designated area (corrected during inspection); and equipment and utensils
are not being air dried.
Falcon Family Restaurant, 1021 S. Main St., Bowling Green, had three critical and one non-critical
violation during a complaint inspection.
The owners stated that no other customer complaints have come in regarding illness.
Critical, which were all corrected during inspection, included handwashing sink is being used for
purposes other than handwashing; food in contact with the bottom of a container; and
temperature-controlled foods not being cold held at the proper temperature.
Non-critical included improper use and/or maintenance of wiping cloths (corrected during inspection).
Ken’s Flower Shops, Inc., 140 W. South Boundary St., Perrysburg, had one critical offense, which was
unapproved chemical sanitizer used on food contact surfaces.
The following inspections were conducted March 27.
Miss Lily’s, 24174 Front St., Grand Rapids, had one non-critical violation during a follow-up inspection,
which was service sink or curbed cleaning facility was not provided and/or conveniently located.
Dollar General, 18300 Pemberville Road, Pemberville, had two non-critical offenses, which were facility
did not have written procedures for vomiting or diarrheal events, and the inspector observed some dented
food cans during inspection. Both were corrected during inspection.
Olde Gilead Country Store, 24139 Front St., Grand Rapids, had two non-critical violations, which included
inadequate number of handwashing sinks, and three-compartment sink not available for manually washing,
rinsing, and sanitizing of utensils.
Blue Bakery, 24125 Front St., Grand Rapids, had one critical and two non-critical offenses.
Critical was the person in charge did not demonstrate knowledge of relationship between food safety and
proper equipment (the person in charge stated gloves are not always worn when handling ready to eat
bakery items).
Non-critical was equipment not approved by a recognized testing agency, and floors, walls, and/or
ceilings were not constructed of approved materials.
Dollar Tree Store, 2676 Woodville Road, Northwood, had four non-critical violations, which included outer
opening not protected; damaged, spoiled and/or recalled products not segregated from food, equipment,
utensils, linens and single-service and single-use articles (corrected during inspection); improper
storage of food items; and no covered receptacle in women’s restroom.
The following inspections were done on March 28.
BeYoutilful Nutrition, 140 E. Wooster St., 3, Bowling Green, had two non-critical violations, which
included in-use utensils improperly stored (corrected during inspection); and non-food contact surfaces
not easily cleanable
Rite Aid, 722-740 S. Main St., Bowling Green, had one critical violation, which was improper display of
poisonous or toxic materials offered for retail sale.
The Baker’s Kitchen by Thrush’s, 210 Louisiana Ave., Perrysburg, had one critical and two non-critical
offenses.
Critical was employees are not informed in a verifiable manner of their responsibility to report
information about their health.
Non-critical included improper storage of food items (corrected during inspection); and plumbing system
not properly maintained or repaired.
The following inspection were conducted March 29.
Subway Sandwiches & Salads, 10677 Fremont Pike, Suite D, Perrysburg, had two critical and five
non-critical offenses.
Critical were equipment food-contact surfaces or utensils are unclean, and temperature-controlled foods
not being cold held at the proper temperature (corrected during inspection).
Non-critical included no written procedures for vomiting or diarrheal events; no covered receptacle in
women’s restroom; floors, walls, and/or ceilings not smooth and easily cleanable; floor and wall
junctures not properly coved or closed and/or floor drains not provided; and physical facilities not
maintained in good repair.
Northwood High School, 700 Lemoyne Road, had one critical offense, which was equipment food-contact
surfaces or utensils are unclean.
The following inspection were done April 2.
St. Rose School, 217 E. Front St., Perrysburg, had one non-critical offense, which was no soap at
handwashing sink(s).
The following inspections were completed April 3.
Rudy’s Dairy Depot, 28572 Oregon Road, Perrysburg, had one critical and one non-critical violation.
Critical was refrigerated, ready-to-eat, temperature-controlled foods not properly date marked (corrected
during inspection).
Non-critical included repairing (walk in freezer condenser is leaking into a container).
Subway, 209 Bierley Ave., Pemberville, had one critical and one non-critical offense.
Critical was handwashing sink is being used for purposes other than handwashing.
Non-critical was personal items stored above food and food items.
Both were corrected during inspection.
Aldi, Inc., 1010 S. Main St., Bowling Green, had one critical and one non-critical offense.
Critical was food not properly protected from contamination by separation, packaging, and segregation.

Non-critical was damaged flooring in the back storage area of the facility
North Baltimore Village Park 1, Girls Field, Taylor Street, North Baltimore, had one critical and two
non-critical offenses.
Critical was temperature-controlled foods were not being held at the proper temperature.
Non-critical were equipment is not approved by a recognized testing agency (repeat); and equipment and/or
components are not maintained in good working order.
Dollar General Store, 1125 S. Main St., Bowling Green, had one critical violation, which was improper
storage of poisonous or toxic materials. The offense was corrected during the inspection.
The following inspections were conducted April 4.
JJ’s Mini Mart, 4101 Woodville Road, Northwood, had two critical and five non-critical violations.
Critical were employees are not informed in a verifiable manner of their responsibility to report
information about their health, and temperature-controlled foods not being cold held at the proper
temperature.
Non-critical include no written procedures for vomiting or diarrheal events (corrected during
inspection); food packaged in the facility not labeled; improper storage of food items; waste
receptacles not covered properly; and floors, walls, and/or ceilings not smooth and easily cleanable
(repeat).
