A Rossford eatery had four critical health code violations during a recent inspection, including using
bare hands to portion out food.
The Wood County Health Department inspected Chili’s Grill & Bar on March 12.
The store, at 9886 Old U.S. 20, also had five non-critical violations during the follow-up inspection,
many which had to do with needed repairs.
Critical offenses included improper bare-hand contact with ready-to-eat foods (observed front of house
team member placing lemons in customer drinks with bare hands, repeat); improper bare-hand contact with
ready-to-eat foods (observed front of the house team member not using gloves to place tortilla chips in
guacamole before serving to customer); temperature-controlled foods not being cold held at the proper
temperature (tomatoes and bagged sausage, repeat); and temperature-controlled foods not being cold held
at the proper temperature (lettuce, salsa).
Non-critical included non-food contact surfaces of equipment are unclean; facility walls and ceiling
tiles not maintained clean; studs, joists, and/or rafters exposed in areas subject to moisture; floor
and wall junctures not properly covered or closed and/or floor drains not provided; and tile flooring
not maintained in good repair.
The following inspections also were done March 12.
BP/Barney’s Convenience Mart, 26475 Dixie Hwy., Perrysburg, had two critical and three non-critical
violations.
Critical included temperature-controlled foods not being cold held at the proper temperature, and ambient
air and water thermometers are not accurate.
Non-critical included improper storage of food items; cleaning agents and/or sanitizers not available in
the Food Service Operation or Retail Food Establishment; and non-food contact surfaces of equipment are
unclean.
Dollar General Store, 13240 Deshler Road, North Baltimore, had one critical and one non-critical offense.

The critical was facility did not have a signed employee health agreement on file at time of inspection.

The non-critical was facility did not have written procedures for vomiting or diarrheal events.
Both were corrected during the inspection.
Jimmy John’s, 10081 Fremont Pike, Perrysburg, had one critical and one non-critical offense.
Critical was equipment food-contact surfaces or utensils are unclean.
Non-critical was equipment and utensils are not being air dried.
Subway, 1137 Buck Road, Rossford, had three critical and three non-critical violations.
Critical included no person in charge present in food facility during inspection; handwashing sink is
being used for purposes other than handwashing; and equipment food-contact surfaces or utensils are
unclean (repeat).
Non-critical included working food containers not properly labeled; single-service articles or single-use
articles stored in prohibited area; and non-durable equipment observed (repeat).
North Baltimore Convenience Station, 101 S. Main St., North Baltimore, had one critical and two
non-critical offenses.
Critical was employees are not informed in a verifiable manner of their responsibility to report
information about their health (corrected during inspection.
Non-critical were facility did not have written procedures for vomiting or diarrheal events (corrected
during inspection); and facility not maintained clean.
BH&H Snacks & Gas, 210 Front St., Cygnet, had one critical and three non-critical
offenses.
Critical was facility did not have signed copies of employee health agreements on file at time of
inspection (corrected during inspection).
Non-critical included no written procedures for vomiting or diarrheal events (corrected during
inspection); non-contact food surfaces not smooth and easily cleanable; and repairing (missing ceiling
tiles over 3-compartment sink in back room).
The following inspections were completed March 13.
Petro Food Mart, 12906 Deshler Road, North Baltimore, had two non-critical violations, including mops
dried improperly (corrected during inspection), and cleaning – frequency and restrictions (dust build-up
in walk-in cooler).
Holiday Inn French Quarter, 10630 Fremont Pike, Perrysburg, had three non-critical violations including
ceiling tiles not maintained in good repair; utility service lines and pipes improperly exposed or
installed; and ventilation system not maintained.
The inspection was triggered by a March 11 complaint from a caller who said his wife had a meeting in one
of the meeting rooms on east side of building. She didn’t see any mold but could smell it as soon as she
went into the room and her allergies started flaring up.
At the time of inspection, no odor was present in any of the rooms.
The business closed by the end of March.
The following inspections were completed March 14.
The Original Gino’s Pizza, 26597 N. Dixie Hwy. Suite 163, Perrysburg, had two critical and three
non-critical violations.
Critical included temperature-controlled foods not being cold held at the proper temperature (repeat),
and refrigerated, ready-to-eat, temperature-controlled foods not properly date marked.
Non-critical included improper method for cooling temperature-controlled foods; non-durable equipment
observed; and non-food contact surfaces of equipment are unclean.
Get Healthy Nutrition, 126 W. South Boundary St., Perrysburg, had two critical offenses, including no
person in charge present in food facility during inspection, and equipment food-contact surfaces or
utensils are unclean (corrected during inspection).
Panera Bread, 10981 Fremont Pike, Perrysburg, had two critical and two non-critical violations.
Critical were missing and/or incomplete allergen declaration(s), and water source and/or hot water
capacity not sufficient (corrected during inspection).
Non-critical included food employees wearing jewelry on arms or hands during food preparation (corrected
during inspection), and food employee(s) hair not effectively restrained.
The following inspections were done March 15.
City Tap and The Attic, 110 N. Main St., Bowling Green, had three non-critical violations during a
follow-up inspection, including non-food contact surfaces of equipment are unclean; facility not
maintained clean; and physical facilities not maintained in good repair.
McDonald’s, 10163 Fremont Pike, Perrysburg, had one critical and two non-critical violations during a
follow-up inspection.
Critical was equipment food-contact surfaces or utensils are unclean.
Non-critical was non-durable equipment observed, and non-food contact surfaces of equipment are unclean.

Dollar General Store, 30500 Drouillard Road, Walbridge, had one critical and two non-critical offenses.

Critical was employees are not informed in a verifiable manner of their responsibility to report
information about their health (corrected during inspection).
Non-critical included no written procedures for vomiting or diarrheal events, and no covered trash
receptacle in women’s restroom.
The following inspection were done March 18.
Quick Stop, 402 E. Wooster St., Bowling Green, had two non-critical violations, including non-food
contact surfaces not easily cleanable, and no protective shielding on lights.
CVS/Pharmacy, 212 E. Wooster St., Bowling Green, had two non-critical offenses, including outer opening
not protected, and unclean non-food contact surfaces (repeat).
BG Beverage Center, 811 Haskins Road, Bowling Green, had two non-critical offenses, including physical
facilities not maintained in good repair, and personal items stored above facility items in the walk-in
cooler.
Dollar Tree Store, 1040 N. Main St. #3, Bowling Green, had one critical violation, which was food
package(s) received in poor condition (repeat).
No offenses were found during inspections of the Wood County Committee on Aging – North Baltimore, 514 W.
Water St.; Dan’s Snacks, 2864 Hangsteller Road, Risingsun; Manny’s Munchies, 721 Jefferson Drive,
Bowling Green; Campus Pollyeyes, 440 E. Court St., Bowling Green, during a follow-up inspection;
Northwestern Water & Sewer District, 108 N. Main St., Walbridge; First Solar, 28101 Cedar Park
Blvd;, Perrysburg, during a follow-up inspection; Zenobia Shrine, 8048 Broadstone Blvd., Perrysburg; and
