TO: ANTHONY TATE, the natural father of Heaven L. Garber, whose usual place of residence is unknown and
cannot with reasonable diligence be ascertained, and/or all interested parties: You are hereby notified
that on the 29th day of May, 2009, Robert D. Trimble filed in the Probate Court of Wood County, Ohio, a
Petition to adopt your minor child born to Tiffany Renee Garber on February 25, 2003, and for a change
of the minor’s name to Heaven Trimble. This Court, located at One Courthouse Square, Bowling Green, Ohio
43402, will hear the petition on the 23rd day of September, 2009, at 9:00 A.M. It is alleged in the
petition, pursuant to R.C. 3107.07, that your consent is not required as you are a parent who has failed
without justifiable cause to provide more than de minimus contact with the minor for a period of at
least one year immediately preceding the filing of the adoption petition and/or placement of the minor
in the home of the petitioner; and, that are a parent who has failed without justifiable cause to
provide for the maintenance and support of the minor as required by law or judicial decree for a period
of at least one year immediately preceding the filing of the adoption petition or the placement of the
minor in the home of the petitioner.
“A FINAL DECREE OF ADOPTION, IF GRANTED, WILL RELIEVE YOU OF ALL PARENTAL RIGHTS AND RESPONSIBILITIES,
INCLUDING THE RIGHT TO CONTACT THE MINOR, AND, EXCEPT WITH RESPECT TO A SPOUSE OF THE ADOPTION
PETITIONER AND RELATIVES OF THAT SPOUSE, TERMINATE ALL LEGAL RELATIONSHIPS BETWEEN THE MINOR AND YOU AND
THE MINOR’S OTHER RELATIVES, SO THAT THE MINOR THEREAFTER IS A STRANGER TO YOU AND THE MINOR’S FORMER
RELATIVES FOR ALL PURPOSES. IF YOU WISH TO CONTEST THE ADOPTION, YOU MUST FILE AN OBJECTION TO THE
PETITION WITHIN FOURTEEN DAYS AFTER PROOF OF SERVICE OF NOTICE OF THE FILING OF THE PETITION AND OF THE
TIME AND PLACE OF HEARING IS GIVEN TO YOU. IF YOU WISH TO CONTEST THE ADOPTION, YOU MUST ALSO APPEAR AT

Notice is hereby given that on August 24, 2009, the Board of Education of Otsego Local School District,
Wood County, Ohio, will hold a public hearing at the Otsego High School Library, Tontogany, Ohio,
commencing at 5:00 p.m., regarding a proposal to direct 0.50 mills of its levy within the ten-mill
limitation to a permanent improvement fund, which may result in an increase in the amount of real
property taxes levied by the Board in tax year 2010 (collection year 2011).

be for hearing before said Court in Bowling Green, Wood County, Ohio, on August 18, 2009, at 3:00 p.m.,
at the Wood County Juvenile Court, 1032 South Dunbridge Road, Bowling Green, Ohio; and James Fisher,
last known address: Montana; is ordered to appear before the Wood County Juvenile Court on said date and
show cause why the complaint’s prayer should not be granted.
