The proposed work consists of the below facilities and other appurtenances necessary for a complete
project:
The City has pre-purchased selected equipment including the microfiltration system skid, microfiltration
clean-in-place equipment, and reverse osmosis skid. The successful contractor will be required to
coordinate with each of these equipment suppliers in order to deliver, unload, install, anchor and
connect each of these skids under the supervision of the respective supplier. The Contractor shall be
responsible for coordination with the membrane equipment suppliers in order to provide the materials,
labor and equipment to install the interconnects needed for each membrane skid, cleaning system, etc.

In addition to installation of the microfiltration and membrane equipment systems, the Contractor is
responsible for construction modifications to the existing Bowling Green Water Treatment Plant buildings
and existing plus new emergency power system as described on the Drawings and specified herein.
The Contractor shall be responsible for the civil work associated with the new Reverse Osmosis building
addition that will house the reverse osmosis membrane skids, reverse osmosis cleaning system,
microfiltration cleaning system, new electrical equipment and other equipment as necessary for the full
functioning operation of the membrane treatment addition.
The provision of materials, installation and testing of the raw water main shall be the responsibility of
the Contractor. The existing water intake structure shall be modified to accommodate the new raw water
main as required for the membrane feed.
The contract documents, including drawings and specifications, are on file at the office of the Utilities
Director, City Administrative Services Building, 304 North Church Street, Bowling Green, Ohio. An
electronic pdf set may be obtained at no cost.
This procurement is subject to the EPA policy of encouraging the participation of small business in rural
areas (SBRAs).
All bids shall be signed and submitted on the blanks which are bound in the specifications. Bid blanks
shall not be removed from the specifications. Bids shall state the unit prices on the blanks provided
therefore; shall be enclosed in a sealed envelope marked – INTEGRATED MEMBRANE WTP
ADDITION
– and addressed to the Utilities Director, City Administrative
Services Building, 304 North Church Street, Bowling Green, Ohio 43402.
Each bid must contain the name of every interested person, firm or corporation and be accompanied by a
bid guaranty to the City of Bowling Green as described in the contract documents.
A bid guaranty may be of two (2) forms:
1. A Bid Guaranty in the amount of 10% of the bid and Contract Bond in the
amount of 100% of the bid using the forms provided in the specifications. (The amount of the bid does
NOT have to appear on this form.

2. A certified check, cashier’s check or letter of credit in favor of the
City of Bowling Green in the amount of 10% of the bid. If the contact is awarded, a Contract Bond will
be required, which is a 100% payment and performance bond.
Bonds and checks of unsuccessful bidders will be returned within ten (10) days after the entering into of
a contact between the City and the successful bidder.
The Successful Bidder will be required to submit an affidavit to the City Finance Director that he has no
delinquent personal property taxes charged against him on the general tax list of personal property in
Wood County, Ohio, at the time this Bid was submitted.
Bids received after the scheduled bid opening date and time, or not accompanied by a satisfactory bid
guaranty, will neither be read nor considered.
The Successful Bidder and any and all Sub-Contractors must comply with the provisions of the Ohio Revised
Code, Chapter 4115, and will be required to enter into a separate Public Improvement Agreement with the
OWNER prior to the execution of the Contract Documents. A sample copy of said Public Improvements is
included in the Instruction to Bidders.
The Successful Bidder will not discriminate against any employee or applicant for employment because of

affirmative action to ensure that applicants for employment are considered for employment and that

and Groups:
On or about but not before, July 6, 2009 the City of Bowling Green will request the State of Ohio to
release Federal funds under Title XII of Division A of the American Recovery and Reinvestment Act (ARRA)
of 2009 (Public Law 111-005, approved February 17, 2009) to carry out the CDBG program under Title I of
the Housing and Community Development Act of 1974 (42 U.S.C. 5301, et seq.) (the HCDA) on an expedited
basis. These funds will be distributed to grantees that received CDBG funding in Fiscal Year (FY) 2008
in accordance with the provisions of 42 U.S.C. 5306. The grant program under Title XII is commonly
referred to as the CDBG Recovery (CDBG-R) program.
Following substantial amendment to the City’s FY 2008 CDBG Annual Action Plan, ARRA funding will be used
for the following activities:
Administration, rental housing rehabilitation, mobile home repair, elderly emergency home repair, B.G.
Transit program support and tree planting. These are single year projects that will be located in the
City of Bowling Green, Wood County, Ohio. The estimated cost of the projects is $81,521.
The Purpose and Nature of the above projects is to improve the quality of life in the community of
Bowling Green by increasing or retaining jobs and improving housing, especially for low-income
individuals.
Environmental Review Records (ERR) for each of the projects listed above have been conducted by the City
of Bowling Green. The ERR’s document the environmental reviews of the projects. The ERR’s are on file
and available for the public’s examination and copying, upon request, between the hours of 8:00 a.m. to
4:30 p.m., Monday through Friday at the above address.
The City of Bowling Green plans to undertake the projects described above with the Federal funds cited
above. Any interested person, agencies and/or groups, who have any comments regarding the environment,
are invited to submit written comments for consideration to the City of Bowling Green, Grants
Administrator at the address above prior to July 6, 2009.
The City of Bowling Green is certifying to the State of Ohio, that the City and John B. Quinn, in his
official capacity as Mayor, consent to accept the jurisdiction of Federal courts if an action is brought
to enforce responsibilities in relation to environmental reviews, decision-making, and action; and that
these responsibilities have been satisfied.
The legal effect of the certification is that upon its approval, the City of Bowling Green may use the
Federal funds, and the State of Ohio will have satisfied its responsibilities under the National
Environmental Policy Act of 1969 as amended.
The State of Ohio will accept an objection to its approval of the release of funds and acceptance of the
certification only if it is on one of the following grounds: (a) the certification was not, in fact,
executed by the responsible entity’s Certifying Officer; (b) the responsible entity has failed to make
one of the two findings pursuant to Section 58.40 or to make the written determination required by
section 58.35, 58.47, or 58.53 for the project, as applicable; c) the responsible entity has omitted one
or more of the steps set forth at subpart E of 24 CFR Part 58 for the preparation, publication and
completion of an Environmental Assessment; d) the responsible entity has omitted one or more of the
steps set forth at Subparts F and G of 24 CFR Part 58 for the conduct, preparation, publication and
completion of an Environmental Impact Statement, e) the recipient has committed funds or incurred costs
not authorized by 24 CFR Part 58 before release of funds and approval of the environmental certification
by State, or f) another federal agency acting pursuant to 40 CFR Part 1504 has submitted a written
finding that the project is unsatisfactory from the standpoint of environmental quality.
Written objections must be prepared and submitted in accordance with the required procedure (24 CFR Part
58), and must be addressed to the: State of Ohio: Environmental Officer, Office of Housing and Community
Partnerships; P.O. Box 1001; Columbus, Ohio 43216-1001.
The State of Ohio will not consider objections to the Release of Funds on bases other than those stated

Each proposal shall be accompanied by the STATE FORM OF COMBINATION BIDDING AND CONTRACT BOND in the sum
of 100% of the amount of the bid or a certified check in the amount of 10% of the bid, or a 10% Bid Bond
submitted on AIA Document A310. Bidders not using the Combination Bidding and Contract Bond must obtain
a 100% Payment and Performance Bond prior to the award of a contract if they are the successful bidder.
Bond amount is to include any add alternates.
Attention is called to the Special Requirements relative to Hours of Employment, and Rules and
Regulations on Equal Employment Opportunity. This project is exempt from prevailing wage rate
requirements.
A PRE-BID MEETING will be held on Thursday, June 25, 2009 at 2:00 pm, local time, to receive any
questions pertaining to the project. The meeting will be held at the shelter house located on the
project site. Enter the site immediately north of the District Transportation Department, 28546 Lime
City Road, Perrysburg, Ohio.
No Bidder may withdraw its bid within sixty (60) days after the time of the bid opening. The Board of
Education reserves the right to reject each and every bid and to waive informalities, irregularities,
and errors in the bidding to the extent permitted by law, including the right to extend the date and
time for receipt of bids; to reject any or all bids, and to conduct such investigation as necessary to
determine the responsibility of a bidder.

BIDDERS

Notice is hereby given that sealed bids will be received until 10:00 a.m., on the 9th day of July, 2009,
by the Board of County Commissioners of Wood County, Ohio, at its office in the Wood County Office
Building, Bowling Green, Ohio, and immediately opened thereafter.
Such proposals as herein concerned shall be for the following described:
“Village of Weston, Drainage and Street Improvements, Walnut Street and Broadway
Avenue”.

All bids shall be sealed and plainly marked: “Village of Weston”
A bid bond or cashier’s check in the amount of five percent (5%) of the total bid amount shall accompany
each proposal submitted. Upon entering into a contract with the Owner, the contractor must then file a
bond for the amount of the contract, and the check will be returned to the successful and unsuccessful
bidders when the contract is executed.
The successful bidder will be required to sign an affidavit of contractor or supplier of non-delinquency
of personal property taxes in accordance with Section 5719.042, Revised Code of Ohio and an affidavit
Regarding Child Support, pursuant to the Board of County Commissioners Resolution No. 92-2041.
All waste materials generated from this project must be taken to the Wood County Landfill for disposal

Attention of bidders is particularly called to the requirements as to conditions of employment to be
observed and minimum wage rates to be paid under the contract, Section 3, Segregated Facility, Section
109 and E.O. 11246.

Bidding blanks and specifications for the same shall be on a prescribed blank and obtained by contacting
the Clerk of the Board of County Commissioners at the Wood County Office Building. All bids shall be
properly signed by an authorized representative of the bidder.
Ohio Revised Code Section 3517.13 I(3) and J(3) requires that no agency or department of this state of
any political subdivision shall enter into any contract for the purchase of good costing more than five
hundred dollars of services costing more than five hundred dollars with a corporation, individual,
partnership or other unincorporated business, association, including, without limitation, a professional
association organized under Chapter 1785 of the Revised Code, estate, or trust unless the contract
includes a certification that the individuals named in Revised Code Sections 3517.13(I)(1) and (J)(1)
are in compliance with the aforementioned provisions. The bidder is required to complete the affidavit
provided. Failure to submit the required form with the proposal/bid package could deem the bidder’s
response to be non-responsive and disqualified from receiving further consideration.
The Board of County Commissioners may reject any or all of the bids. The Board of County Commissioners
reserves the right to waive any irregularities in the form of the bid that do not affect or destroy
competitive bidding. In no case will an award be made until all necessary investigations are made as to
the qualifications of the bidder to whom it is proposed to award the contract.
