Wood County Health Department conducts inspections

A Chinese restaurant in Perrysburg had more than 20 violations during a recent health inspection.

The Wood County Health Department conducted a follow-up inspection at Yum Yummy, 10677 Fremont Pike, on Jan. 19 and found eight critical and 16 non-critical violations.

Eleven were repeat offenses.

Critical were written procedures and plans are not maintained and implemented; food not properly protected from contamination by separation, packaging, and segregation (repeat); equipment food-contact surfaces or utensils are unclean (repeat); food equipment surfaces not cleaned at required frequency (repeat, corrected during inspection); chlorine sanitizing solution at incorrect temperature and/or concentration (corrected during inspection); ready-to-eat, temperature-controlled food not properly discarded when required (corrected during inspection); refrigerated, ready-to-eat, temperature-controlled foods not properly date marked; and working containers of poisonous or toxic materials not properly labeled (corrected during inspection).

Non-critical were improper method for cooling temperature-controlled foods; working food containers not properly labeled (repeat, corrected during inspection); outer opening not protected (repeat); improper storage of food items (repeat); linens used inappropriately (repeat); improper use and/or maintenance of wiping cloths (repeat); re-use of single-service or single-use articles (repeat); non-food contact surfaces not easily cleanable; non-food contact surfaces not easily cleanable; cutting blocks or boards cannot be effectively cleaned and sanitized; utensils and/or equipment not properly rinsed; warewashing equipment and/or components not cleaned at required frequency (repeat); non-food contact surfaces of equipment are unclean; plumbing system not properly maintained or repaired; unnecessary or nonfunctional items and/or litter on premises (repeat); and no protective shielding on lights.

The following inspections were done Jan. 11.

J Cups, 616 Dixie Hwy., Rossford, had one critical violation, which was ready-to-eat, temperature-controlled food not properly discarded when required. It was corrected during inspection.

The following inspections were done Jan. 12.

Bai Du OH Inc., 580 Craig Drive, Unit 3 & 4, Perrysburg, had eight critical and eight non-critical violations.

Critical were employees are not informed in a verifiable manner of their responsibility to report information about their health; improper washing of hands and arms; food employee(s) not washing hands in approved handwashing sink or automatic handwashing facility; equipment food-contact surfaces or utensils are unclean; temperature-controlled foods not being hot held at the proper temperature (corrected during inspection); refrigerated, ready-to-eat, temperature-controlled foods not properly date marked; improper use of a poisonous or toxic substance; and working containers of poisonous or toxic materials not properly labeled.

Non-critical were in-use utensils improperly stored; linens were not kept clean; re-use of single-service or single-use articles; cooking equipment or pan surfaces unclean; non-food contact surfaces of equipment are unclean; plumbing system not properly maintained or repaired; facility not maintained clean; and physical facilities not maintained in good repair.

The following inspections were done Jan. 13.

Al-Mar Lanes, 1010 N. Main St., Bowling Green, had two critical and five non-critical violations.

Critical were quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness; and refrigerated, ready-to-eat, temperature-controlled foods not properly date marked (corrected during inspection).

Non-critical were employee eating, drinking, or using tobacco in non-designated area; outer opening not protected (repeat); non-food contact surfaces not easily cleanable; non-food contact surfaces of equipment are unclean (repeat); and facility not maintained clean (repeat).

Kentucky Fried Chicken/Long John Silvers, 1020 N. Main St., Bowling Green, had six non-critical violations, which were artificial nails and/or polished nails contacting food with bare hands; equipment components are not intact, tight or properly adjusted; non-food contact surfaces of equipment are unclean (repeat); waste receptacles not covered properly; physical facilities not maintained in good repair (repeat); and facility not maintained clean (repeat).

Hy-Miler, 2488 Oregon Road, Northwood, had two critical and five non-critical violations.

Critical were quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness; and temperature-controlled foods not being cold held at the proper temperature. Both were corrected during inspection.

Non-critical were no soap at handwashing sink(s) (corrected during inspection); temperature-controlled foods not properly thawed; equipment components are not intact, tight or properly adjusted (repeat); non-food contact surfaces lined with absorbent materials (repeat); and physical facilities not maintained in good repair.

The following inspections were done Jan. 17.

Original Royal Pizza, 2038 Tracy Road, Northwood, had one critical and two non-critical offenses.

Critical was improper storage of poisonous or toxic materials. It was corrected during inspection.

Non-critical were non-food contact surfaces of equipment are unclean (repeat); and physical facilities not maintained in good repair (repeat).

During a complaint inspection, Applebee’s Neighborhood Grill & Bar, 3007 Curtice Road, Northwood, had one non-critical offense, which was facility not maintained clean.

A complainant on Jan. 13 said there was mold between the bun rack and the CO2 tank.

The inspector spoke with the manager, who said the air conditioning was located in the ceiling by the CO2 tank and the facility believes there is a leak. Mold was not observed at time of inspection, but debris and standing water were observed on the floor below the dry storage and pop boxes.

The following inspections were done Jan. 18.

Holiday Inn Express, 12710 Roachton Road, Perrysburg, had two critical and one non-critical offense.

Critical were written procedures and plans are not maintained and implemented; and temperature-controlled foods not being hot held at the proper temperature. Both were corrected during inspection.

Non-critical was no written procedures for time as a public health control (repeat).

Marco’s Pizza, 629 W. South Boundary St., Perrysburg, had one critical and two non-critical offenses.

Critical was no air gap or approved backflow prevention device on the plumbing system.

Non-critical were improper storage of food items (repeat, corrected during inspection); and facility walls not maintained clean.

Cinco De Mayo Mexican Bar & Grill, 1213 Schreier Road, Rossford, had two critical and two non-critical offenses.

Critical were food not properly protected from contamination by separation, packaging, and segregation; and temperature-controlled foods not being cold held at the proper temperature (repeat). Both were corrected during inspection.

Non-critical were non-food contact surfaces constructed of unapproved materials; and floors, walls, and/or ceilings not smooth and easily cleanable.

During a follow-up inspection, S&G, 26475 Dixie Hwy., Perrysburg, had one non-critical offense, which was facility ceiling and wall not maintained clean.

The following inspections were done Jan. 19.

Otterbein Portage Valley, 20311 Pemberville Road, Pemberville, had one critical and one non-critical offense.

Critical was temperature-controlled foods not being cold held at the proper temperature (repeat). It was corrected during inspection.

Non-critical was food service operation did not submit facility layout and equipment specifications.

China Village, 134 W. Wooster St., Bowling Green, had two critical and eight non-critical violations.

Critical were temperature-controlled foods not being cold held at the proper temperature (corrected during inspection); and direct connection between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed (repeat).

Non-critical were facility does not have an employee with manager certification in food protection (repeat); handwashing sink water below 100°F (corrected during inspection); inadequate number of hand washing sinks (repeat); re-use of single-service or single-use articles; equipment not approved by a recognized testing agency (repeat); non-food contact surfaces of equipment are unclean (repeat); physical facilities not maintained in good repair (repeat); and facility not maintained clean (repeat).

During a follow-up inspection, Wendy’s Old Fashioned Hambrgs., 1504 E. Wooster St., Bowling Green, had two non-critical offenses, which were non-food contact surfaces of equipment are unclean; and physical facilities not maintained in good repair.

Starbucks Coffee, 1560 E. Wooster St., Bowling Green, had two non-critical offenses, which were irreversible registering temperature indicator not readily accessible for hot water mechanical warewashing operations (repeat); and facility not maintained clean.

Frobose Market IGA, 209 Bierley Ave., Pemberville, had two critical and two non-critical offenses.

Critical were food not properly protected from contamination by separation, packaging, and segregation; and ready-to-eat, temperature-controlled food not properly discarded when required. Both were corrected during inspection.

Non-critical were equipment components are not intact, tight or properly adjusted (repeat); and physical facilities not maintained in good repair (repeat).

Pisanello’s Pizza, 203 N. Main St., Bowling Green, had two non-critical offenses, which were non-food contact surfaces of equipment are unclean; and physical facilities not maintained in good repair (repeat).

Waffle House, 1548 E. Wooster St., Bowling Green, had one critical and one non-critical offense.

Fairfield Inn & Suites, 1544 E. Wooster St., Bowling Green, had two critical and two non-critical offenses.

Non-critical were inadequate equipment available for cooling, heating or holding food; and working food containers not properly labeled (corrected during inspection).

During a follow-up inspection, BG Burgers, 1424 E. Wooster St., Bowling Green, had one critical and two non-critical offenses.

Critical was temperature-controlled foods not being hot held at the proper temperature.

Non-critical were equipment components are not intact, tight or properly adjusted; and physical facilities not maintained in good repair.

SamB’s, 146 N. Main St., Bowling Green, had four critical and four non-critical violations.

Critical were food not properly protected from contamination by separation, packaging, and segregation (corrected during inspection); temperature-controlled foods not being cold held at the proper temperature (repeat, corrected during inspection); ready-to-eat, temperature-controlled food not properly discarded when required (corrected during inspection); and refrigerated, ready-to-eat, temperature-controlled foods not properly date marked (repeat).

Non-critical were temperature-controlled foods not properly thawed (corrected during inspection); equipment not approved by a recognized testing agency (repeat); ventilation hood system not sufficient to prevent grease or condensation build-up; and food service operation did not submit facility layout and equipment specifications (repeat).

Crimson Cup, 1 Olscamp Hall, Bowling Green, had two critical and one non-critical offense.

Critical were food equipment surfaces not cleaned at required frequency; and temperature-controlled foods not being cold held at the proper temperature. Both were corrected during inspection.

Non-critical was in-use utensils improperly stored. It was corrected during inspection.

The following inspection was done Jan. 20.

During a follow-up inspection, Thrive Childcare, 1134 Professional Drive, Perrysburg, had three non-critical offenses, which were improper storage of food items; clean equipment and utensils not stored in a self-draining position and covered or inverted; and equipment not approved by a recognized testing agency.

Zero violations were found at Kim’s Kakes LLC, 480 Dixie Hwy., Rossford; First United Methodist Child Learning Center, 1526 E. Wooster St., Bowling Green; Panera Bread, 10981 Fremont Pike, Perrysburg; Bob Evans Restaurants LLC, 10770 Fremont Pike, Perrysburg; Stroh – Wooster St. Station, 1535 E. Wooster St., Bowling Green; Stroh – The Perch, 1535 E. Wooster St., Bowling Green; Waffle House, 26046 N. Dixie Hwy., Perrysburg; and Anderson Arena Concession Stand, 1022 Ridge St., Bowling Green.
