All materials to be furnished must
comply with State Highway Department Specifications for Bituminous
Material, Section 702, et. seq.
It is understood that a vendor may
bid, and will be considered for the furnishing or any part of the
300,000 gallons, in transport quantities, that the vendor is reasonably
sure can be furnished.
Bids submitted shall be in letter form, showing
a unit price per gallon for supplying such needed materials from the
types as hereinafter listed and the same to be shipped F.O.B. to job
site, as may be designated in Wood County, Ohio; also quote, if
available cost on basis of plant pick up, it being understood that
prices submitted shall include any and all costs of insulated
transports; also terms regarding demurrage charges, and all invoices for
deliveries shall be on the basis of one (1) invoice for total load.
Consideration as to the net cost to Wood County will be specific
consideration in this matter.

Bids will be received by the Middleton Township Trustees at the

The estimate of probable cost
for this project is $682,000.00, broken down as General Contract

Bidders shall not be responsible for
paying sales taxes on materials incorporated into the structure.

The successful bidder will be required to pay not less than the minimum

Bids received after the scheduled bid opening date and

The Owner reserves the right to reject

Bids will be received by the Middleton Township Trustees at the
offices of the Architect, Poggemeyer Design Group, 1168 North Main
Street, Bowling Green, Ohio until 2:00 p.m. on Thursday, February 27, 2014,
and at that time and place will be publicly opened and read aloud.
The work will consist of new construction of a 3400 SF wood framed,
one-story story administration building.
The estimate of probable cost
for this project is $682,000.00, broken down as General Contract
$505,000; Plumbing Contract $35,000.00; Mechanical Contract $58,000; and
Electrical Contract $84,000.
Bidders shall not be responsible for
paying sales taxes on materials incorporated into the structure.
The
contract documents, including plans and specifications, are on file at

The bid guaranty may be of two
forms:
1. A Bid Guaranty and Contract Bond using the form in the
Contract Documents.
(The amount of the bid does NOT have to appear on
this form.)
2. A certified check, cashier’s check or letter of credit
in favor of the Middleton Township, in the amount of ten percent (10%)
of the bid.
If the contract is awarded a Contract Bond will be
required, which is a one hundred percent (100%) payment and performance
bond.
Bidders shall submit with their bid the affidavit required under
the Ohio Revised Code, Section 5719.042 that the bidder was not charged
with any delinquent personal property taxes in Wood County, Ohio.
The successful bidder will be required to pay not less than the minimum
wage rates established by the Department of Industrial Relations of the
State of Ohio in accordance with all provisions of the Prevailing Wage
Act of the State of Ohio, ORC Sections 4115.03 and 4115.16 and related
requirements.
Bids received after the scheduled bid opening date and
time, or not accompanied by a satisfactory bid bond or check, will
neither be read nor considered.
The Owner reserves the right to reject
any and all bids and to waive any irregularity in any bid and to
determine the lowest and best bidder.
In no case will an award be made
until all necessary investigations are made as to the qualifications of
the bidder to whom it is proposed to award the contract.
No bidder may
withdraw his bid for a period of sixty (60) days after the scheduled
