Wood County Health Dept. conducts inspections

The Wood County Health Department conducted inspections in Perrysburg, Northwood, Grand Rapids, Rossford and Bowling Green at the end of February.

The following inspections were done Feb. 21.

During a follow-up inspection, Pizza Pub 516 of Perrysburg, 27072 Carronade Drive, Unit K, had nine non-critical violations, which were inadequate number of handwashing sinks; food employees wearing jewelry on arms or hands during food preparation; food employee(s) not wearing a hair restraint; fixed equipment not properly sealed or spaced for cleaning; cutting blocks or boards cannot be effectively cleaned and sanitized; non-food contact surfaces of equipment are unclean; plumbing system not properly maintained or repaired; physical facilities not maintained in good repair; and light intensity less than 50 foot candles in required areas

Zingo’s Mediterranean, 106 Louisiana Ave., Perrysburg, had five critical and two non-critical offenses.

Critical were handwashing sink is being used for purposes other than handwashing; quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness (corrected during inspection); temperature-controlled foods not being hot held at the proper temperature (corrected during inspection); temperature-controlled foods not being cold held at the proper temperature (repeat, corrected during inspection); and working containers of poisonous or toxic materials not properly labeled (corrected during inspection).

Non-critical were non-food contact surfaces not easily cleanable; and non-food contact surfaces of equipment are unclean.

Edible Arrangements, 26567 N. Dixie Hwy., Perrysburg, had two critical offenses, which were employees are not informed in a verifiable manner of their responsibility to report information about their health; and refrigerated, ready-to-eat, temperature-controlled foods not properly date marked.

The Clay Pot, 182 S. Main St., Bowling Green, had one critical offense, which was equipment food-contact surfaces or utensils are unclean.

During a follow-up inspection, Sterling’s Amish Deli, 133 E. Wooster St., Bowling Green, had three non-critical offenses, which were food employee(s) not wearing a hair restraint; cutting blocks or boards cannot be effectively cleaned and sanitized; and physical facilities not maintained in good repair.

Promedica Skilled Nursing and Rehabilitation (Perrysburg), 10540 Fremont Pike, had one critical offense, which was insufficient air gap between the flood rim and the water supply inlet (repeat).

The Row, 130 E. Wooster St., Bowling Green, had one non-critical offense, which was non-food contact surfaces of equipment are unclean.

The following inspections were done Feb. 22.

Trotter’s Tavern, 119 N. Main St., Bowling Green, had four non-critical offenses, which were non-food contact surfaces not easily cleanable (repeat); non-food contact surfaces of equipment are unclean; physical facilities not maintained in good repair (repeat); and facility not maintained clean.

Bangkok Café, 3401 Woodville Road Suite 1/2, Northwood, had four critical and four non-critical violations.

Critical were food not properly protected from contamination by separation, packaging, and segregation; food not properly protected from contamination by separation, packaging, and segregation; temperature-controlled foods not being cold held at the proper temperature (repeat); and improper storage of poisonous or toxic materials. All were corrected during inspection.

Non-critical were improper storage of food items (corrected during inspection; facility not maintained clean; physical facilities not maintained in good repair; and dressing rooms or lockers not being properly used. All were repeat offenses.

During a complaint inspection, Super 8 Motel, 3491 Latcha Road, Millbury, had four non-critical offenses, which were inadequate number of handwashing sinks; equipment not approved by a recognized testing agency (repeat); physical facilities not maintained in good repair; and food service organization did not submit facility layout and equipment specifications.

A complaint was received Feb. 21 that there was no running water in the facility.

The inspector learned the new three-bay sink was recently installed and does not have running water. Hot water was observed in the two-bay sink located in the laundry room. Per maintenance, the hot water was out for a couple of hours the day prior, but they were able to get it up and running again.

During a follow-up inspection, Pizza Hut, 1131 Louisiana Ave., Perrysburg, had seven non-critical offenses, which were facility does not have an employee with manager certification in food protection (repeat); outer opening not protected (repeat); artificial nails and/or polished nails contacting food with bare hands; improper storage of single-service and single-use articles; warewashing equipment and/or components not cleaned at required frequency (repeat); non-food contact surfaces of equipment are unclean (repeat); and facility not maintained clean (repeat).

The following inspections were done Feb. 23.

Fernando’s, 24129 Front St., Grand Rapids, had one critical offense, which was chlorine sanitizing solution at incorrect temperature and/or concentration. It was corrected during inspection.

1st Wok, 10093 Fremont Pike, Perrysburg, had five non-critical offenses, which were improper storage of food items; re-use of single-service or single-use articles; equipment components are not intact, tight or properly adjusted; non-food contact surfaces of equipment are unclean; and physical facilities not maintained in good repair.

During a follow-up inspection, Miss Lily’s, 24174 Front St., Grand Rapids, had seven non-critical violations, which were non-food contact surfaces not easily cleanable; non-food contact surfaces not easily cleanable; service sink or curbed cleaning facility not provided and/or conveniently located; facility not maintained clean; physical facilities not maintained in good repair; carpet installed in unapproved areas; and food service operation did not submit facility layout and equipment specifications.

St. Clare Commons, 12469 Five Point Road, Perrysburg, had three critical and three non-critical offenses.

Critical were chlorine sanitizing solution at incorrect temperature and/or concentration (repeat); temperature-controlled foods not being cold held at the proper temperature (repeat, corrected during inspection); and improper storage of poisonous or toxic materials (corrected during inspection).

Non-critical were handwashing sink water below 100°F; inadequate equipment available for cooling, heating or holding food (repeat); and non-food contact surfaces of equipment are unclean (repeat).

Tea Tree Asian Bistro, 4100 Chappel Drive, Perrysburg, has six critical and seven non-critical violations.

Critical were written procedures and plans are not maintained and implemented (repeat); food employee(s) did not wash hands when required (corrected during inspection); handwashing sink not accessible (repeat); records for freezing of fish for parasite destruction not retained; temperature-controlled foods not being cold held at the proper temperature (corrected during inspection); and food contact surfaces not easily cleanable.

Non-critical were no soap at handwashing sink(s) (corrected during inspection); temperature-controlled foods not properly thawed (repeat); working food containers not properly labeled (repeat); improper storage of food items; no sanitizer test kit available for chlorine sanitizer; non-food contact surfaces of equipment are unclean; and floors, walls, and/or ceilings not smooth and easily cleanable.

Nagoya Japanese Steakhouse, 6190 Levis Commons Blvd., Perrysburg, had 10 critical and four non-critical violations.

Critical were food not properly protected from contamination by separation, packaging, and segregation (corrected during inspection); food not properly protected from contamination by separation, packaging, and segregation (corrected during inspection); quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness (corrected during inspection); equipment food-contact surfaces or utensils are unclean (repeat, corrected during inspection); temperature-controlled foods not being cold held at the proper temperature (repeat, corrected during inspection); ready-to-eat, temperature-controlled food not properly discarded when required (repeat, corrected during inspection); refrigerated, ready-to-eat, temperature-controlled foods not properly date marked (repeat); improper use of time as a public health control (four hours); working containers of poisonous or toxic materials not properly labeled (corrected during inspection); and improper storage of poisonous or toxic materials (repeat, corrected during inspection).

Non-critical were improper method for cooling temperature-controlled foods (corrected during inspection); temperature-controlled foods not properly thawed (repeat); unapproved alternative equipment for manually warewashing (corrected during inspection); and physical facilities not maintained in good repair (repeat).

Harbor Town Tavern, 7010 Lighthouse Way, Perrysburg, had five critical and five non-critical violations.

Critical were person in charge not ensuring employees are properly sanitizing equipment and utensils and monitoring of sanitizer parameters; food not properly protected from contamination by separation, packaging, and segregation; temperature-controlled foods not being cold held at the proper temperature; improper storage of poisonous or toxic materials; food contact surfaces not easily cleanable. All were corrected during inspection.

Non-critical were inadequate equipment available for cooling, heating or holding food; non-food contact surfaces not easily cleanable; irreversible registering temperature indicator not readily accessible for hot water mechanical warewashing operations; non-food contact surfaces of equipment are unclean; and facility not maintained clean (repeat).

The following inspections were conducted Feb. 24.

During a follow-up inspection, Bob Evans Restaurants LLC, 1313 Levis Commons Blvd., Perrysburg, had two critical and two non-critical offenses.

Critical were ready-to-eat, temperature-controlled food not properly discarded when required; and refrigerated, ready-to-eat, temperature-controlled foods not properly date marked. Both were repeat offenses.

Non-critical were non-food contact surfaces of equipment throughout the facility are unclean; and facility not maintained clean. Both were repeat offenses.

Meijer, 10055 Olde U.S. 20, Rossford, had three critical and two non-critical offenses.

Critical were food not properly protected from contamination by separation, packaging, and segregation; improper storage of poisonous or toxic materials; and food contact surfaces not easily cleanable. All were corrected during inspection.

Non-critical were non-food contact surfaces of equipment are unclean; and physical facilities not maintained in good repair.

Poco Perrysburg, Inc., 3155 Chappel Drive, had three critical and eight non-critical violations.

Critical were food in contact with an unclean surface or linens (repeat); equipment food-contact surfaces or utensils are unclean (repeat); and improper storage of poisonous or toxic materials. All were corrected during inspection.

Non-critical were improper storage of food items; cutting blocks or boards cannot be effectively cleaned and sanitized; no sanitizer test kit available; non-food contact surfaces of equipment are unclean; cooking equipment or pan surfaces unclean; physical facilities not maintained in good repair; floor and wall junctures not properly coved or closed and/or floor drains not provided; and facility not maintained clean. All were repeat offenses.

During a follow-up inspection, Stella’s, 104 Louisiana Ave., Perrysburg, had two non-critical offenses, which were fixed equipment not properly sealed or spaced for cleaning; and non-food contact surfaces of equipment are unclean.

Reset and Cameos Pizza, 27250 Crossroads Pkwy., Rossford, had five critical and eight non-critical violations.

Critical were food equipment surfaces not cleaned at required frequency (corrected during inspection); quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness (repeat); temperature-controlled foods not being hot held at the proper temperature (corrected during inspection); temperature-controlled foods not being cold held at the proper temperature (corrected during inspection); and improper storage of poisonous or toxic materials (corrected during inspection).

Non-critical were facility does not have an employee with manager certification in food protection (repeat); no soap at handwashing sink(s) (corrected during inspection); improper storage of food items (repeat); food employee(s) not wearing a hair restraint (repeat); equipment and utensils are not being air dried; non-food contact surfaces of equipment are unclean (repeat); facility not maintained clean (repeat); and food service operation did not have a person in charge per shift with the person in charge certification in food protection (repeat);

The Flying Bean, 2130 Preston Parkway, Perrysburg, had one non-critical offense, which was floors, walls, and/or ceilings not smooth and easily cleanable (repeat).

2523 Oregon LLC, 2523 Oregon Road, Northwood, had two critical and four non-critical offenses.

Critical were quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness (corrected during inspection); and temperature-controlled foods not being cold held at the proper temperature. Both were repeat offenses.

Non-critical were equipment and utensils are not being air dried (repeat); non-food contact surfaces of equipment are unclean (repeat); physical facilities not maintained in good repair (repeat); and food service operation did not have a person in charge per shift with the person in charge certification in food protection.

China East Buffet, Inc. DBA China City, 27072 Carronade Drive, Suite M, had one critical and five non-critical offenses.

Critical was temperature-controlled foods not being cold held at the proper temperature (repeat, corrected during inspection).

Non-critical were working food containers not properly labeled (corrected during inspection); non-food contact surfaces not easily cleanable; non-durable equipment observed (repeat); cooking equipment or pan surfaces unclean; and light intensity less than 10 foot candles in required areas.

Hissho Sushi @ Meijer, 10055 Olde U.S. 20, Rossford, had one non-critical offense, which was non-food contact surfaces not easily cleanable.

Six Fifths, 120 W. South Boundary, Perrysburg, had two critical and two non-critical offenses.

Critical were refrigerated, ready-to-eat, temperature-controlled foods not properly date marked (corrected during inspection); and food contact surfaces not easily cleanable.

Non-critical were in-use utensils improperly stored (corrected during inspection); and cutting blocks or boards cannot be effectively cleaned and sanitized.

The Carranor Hunt & Polo Club, 502 E. Second St., Perrysburg, had six non-critical offenses, which were improper storage of food items; improper storage of cleaned equipment, utensils, and laundered linens; equipment not approved by a recognized testing agency; fixed equipment not properly sealed or spaced for cleaning; non-food contact surfaces of equipment are unclean (repeat); and unnecessary or nonfunctional items and/or litter on premises.

Zero offenses were found at Oaks Dining Center, Bowling Green State University, and Sweetfish – Bubble Tea Poke Bowl, 26597 N. Dixie Hwy., Perrysburg.
