A Northwood hotel had mouse droppings on a prep countertop during a recent health inspection.

The Wood County Health Department on Feb. 8 inspected Baymont Inn & Suites, 2426 Oregon Road, and found four critical and 11 non-critical violations.

Critical were presence of live insects, rodents, and other pests (mice droppings were observed in the kitchen area and on the prep countertop); employees are not informed in a verifiable manner of their responsibility to report information about their health (repeat); equipment food-contact surfaces or utensils are unclean (repeat); and food contact surfaces not easily cleanable (repeat).

Non-critical were no written procedures for responding to vomiting or diarrheal events; no soap at handwashing sink(s); no towels or drying device at the handwashing sink(s); equipment not approved by a recognized testing agency; cutting blocks or boards cannot be effectively cleaned and sanitized; non-food contact surfaces not easily cleanable; no sanitizer test kit available; non-food contact surfaces of equipment are unclean; unnecessary or nonfunctional items and/or litter on premises; mops dried improperly; and physical facilities not maintained in good repair. All were repeat violations except mops improperly dried.

The following inspections were done Feb. 7.

Pollyeyes East Inc. DBA Campus Pollyeyes, 440 E. Court St., Bowling Green, had four non-critical offenses, which were no written procedures for time as a public health control; food employee(s) not wearing a hair restraint; non-food contact surfaces not easily cleanable; and physical facilities not maintained in good repair (repeat).

Bowling Green Country Club, 923 Fairview Ave., had three critical and five non-critical violations.

Critical were food employee(s) did not wash hands when required; chlorine sanitizing solution at incorrect temperature and/or concentration; and ready-to-eat, temperature-controlled food not properly discarded when required. All were corrected during inspection.

Non-critical were facility does not have an employee with manager certification in food protection (repeat); employee eating, drinking, or using tobacco in non-designated area (corrected during inspection); non-food contact surfaces not easily cleanable (repeat); non-food contact surfaces of equipment are unclean (repeat); and facility not maintained clean (repeat).

Kroger N-548, 27322 Carronnade Drive, Perrysburg, had two critical and four non-critical violations.

Critical were employees are not informed in a verifiable manner of their responsibility to report information about their health; and temperature-controlled foods not being cold held at the proper temperature (corrected during inspection).

Non-critical were improper storage of food items (repeat); non-food contact surfaces not easily cleanable; non-food contact surfaces of equipment are unclean (repeat); and physical facilities not maintained in good repair.

During a follow-up inspection, Yala Kol Express, 27151 D Crossroads Pkwy., Rossford, had six non-critical offenses, which were re-use of single-service or single-use articles; non-food contact surfaces not easily cleanable; equipment components are not intact, tight or properly adjusted; non-food contact surfaces of equipment are unclean; physical facilities not maintained in good repair; and food service operation did not have a person in charge per shift with the person in charge certification in food protection.

Total Sports Grill, 10020 S Compass Drive, Rossford, had two critical and two non-critical offenses.

Critical were equipment food-contact surfaces or utensils are unclean; and working containers of poisonous or toxic materials not properly labeled (corrected during inspection);

Non-critical were facility does not have an employee with manager certification in food protection (repeat); and food service operation did not have a person in charge per shift with the person in charge certification in food protection.

Moody’s Coney Island, 2511 Oregon Road, Northwood, had four critical and four non-critical violations.

Critical were food not properly protected from contamination by separation, packaging, and segregation; chlorine sanitizing solution at incorrect temperature and/or concentration; refrigerated, ready-to-eat, temperature-controlled foods not properly date marked; and working containers of poisonous or toxic materials not properly labeled (repeat). All were corrected during inspection.

Non-critical were food employee(s) hair not effectively restrained; non-food contact surface(s) not cleaned at the required frequency; mops dried improperly (corrected during inspection); and facility not maintained clean.

Imaginative Beginnings Inc., 4937 Woodville Road, Northwood, had one critical and one non-critical offense.

Critical was temperature-controlled foods not being cold held at the proper temperature. It was corrected during inspection.

Non-critical was physical facilities not maintained in good repair.

Alehouse Grill, 1234 N. Main St., Bowling Green, had four non-critical offenses, which were non-food contact surfaces not easily cleanable; plumbing system not properly maintained or repaired; physical facilities not maintained in good repair; and facility not maintained clean. All were repeat violations.

During a follow-up inspection, Bai Du OH Inc., 580 Craig Drive, Unit 3 & 4, Perrysburg, had two critical and seven non-critical offenses.

Critical were food not properly protected from contamination by separation, packaging, and segregation; and refrigerated, ready-to-eat, temperature-controlled foods not properly date marked (repeat, corrected during inspection).

The following inspections were done Feb. 9.

Stone Ridge Golf Club, 1553 Muirfield Drive, Bowling Green, had three critical and one non-critical offense.

Critical were quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness; temperature-controlled foods not being cold held at the proper temperature; and improper storage of poisonous or toxic materials. All three were corrected during inspection.

Non-critical was utensils and equipment contacting non-temperature-controlled foods not cleaned at required frequency. It was corrected during inspection.

During a complaint inspection, Snyder’s Restaurant, 24098 Front St., Grand Rapids, had six critical and 15 non-critical violations.

A complaint about the pizza buffet was received Feb. 3. The inspector spoke with the owner regarding complaint. Pizza was observed in enclosed hot holding unit for customer service. A full inspection was conducted, and pizzas were observed holding at proper temperatures.

The Nest Bowling Green, 408 S. Mitchell Road, had one critical offense, which was improper storage of poisonous or toxic materials. It was corrected during inspection.

Madd Jake Ventures LLC, DBA Beckett’s Burger Bar, 163 S. Main St., Bowling Green, had four critical and two non-critical violations.

Critical were food not properly protected from contamination by separation, packaging, and segregation; chlorine sanitizing solution at incorrect temperature and/or concentration; improper reheating of food for hot holding; and improper storage of poisonous or toxic materials. All four were corrected during inspection.

Non-critical was in-use utensils improperly stored (corrected during inspection); and cutting blocks or boards cannot be effectively cleaned and sanitized.

Condado Tacos, 10584 Fremont Pike, Perrysburg, had one critical and one non-critical offense.

Critical was temperature-controlled foods not being hot held at the proper temperature (repeat, corrected during inspection).

Non-critical was equipment and utensils are not being air dried (repeat).
