Wood County Health Department conducts inspections

The Wood County Health Department during the last week of October conducted inspections in Perrysburg, Northwood, Bowling Green and Weston.

The following inspections were done Oct. 24.

Dunkin Donuts/Shree Donuts Inc., 26555 N. Dixie Hwy., Perrysburg, had three non-critical offenses, which were employee food, drink or tobacco in non-designated area (corrected during inspection); food employee(s) not wearing a hair restraint (repeat); and non-food contact surfaces of equipment are unclean (repeat).

China City, 129 N. Main St., North Baltimore, had two critical and eight non-critical violations.

Critical were handwashing sink is being used for purposes other than handwashing (repeat); and temperature-controlled foods not being cold held at the proper temperature. Both were corrected during inspection.

Non-critical were improper storage of cleaned equipment, utensils, and laundered linens; non-food contact surfaces not easily cleanable (repeat); non-durable equipment observed; cooking equipment or pan surfaces unclean; unnecessary or nonfunctional items and/or litter on premises (repeat); facility not maintained clean (repeat); physical facilities not maintained in good repair (repeat); and light intensity less than 50 foot candles in required areas.

Ninja Hibachi Sushi Steakhouse, 1080 S. Main St., Bowling Green, had four critical and two non-critical violations.

Critical were equipment food-contact surfaces or utensils are unclean; chlorine sanitizing solution at incorrect temperature and/or concentration; temperature-controlled foods not being cold held at the proper temperature; and improper storage of poisonous or toxic materials. All were repeat offenses and all were corrected during inspection.

Non-critical were non-food contact surfaces of equipment are unclean (repeat); and employee food items observed over foodservice items in the reach-in cooler and walking cooler (repeat, corrected during inspection).

Buddy’s Place, 100 Main St., Risingsun, had two critical and nine non-critical violations.

Critical were food equipment surfaces not cleaned at required frequency (repeat); and temperature-controlled foods not being cold held at the proper temperature (corrected during inspection).

Non-critical were facility does not have an employee with manager certification in food protection (repeat); inadequate equipment available for cooling, heating or holding food; in-use utensils improperly stored (repeat); non-food contact surfaces constructed of unapproved materials (repeat); equipment components are not intact, tight or properly adjusted (repeat); non-food contact surface(s) not cleaned at the required frequency (repeat); unnecessary or nonfunctional items and/or litter on premises; mops dried improperly (corrected during inspection); and retail food establishment does not comply with Level One Certification requirements (repeat).

The following inspections were done Oct. 25.

Cracker Barrel Old Country Store Inc., 104, 27491 Helen Drive, Perrysburg, had one critical and two non-critical offenses.

Critical was working containers of poisonous or toxic materials not properly labeled. It was corrected during inspection.

Non-critical were non-food contact surfaces of equipment are unclean (repeat); and facility walls and ceiling not maintained clean.

Taco Bell, 25782 N. Dixie Hwy., Perrysburg, had one critical and two non-critical offenses.

Critical was equipment food-contact surfaces or utensils are unclean. It was corrected during inspection.

Non-critical were equipment and utensils are not being air dried prior to stacking; and facility not maintained clean (repeat).

Subway, 10392 Fremont Pike, Perrysburg, had two critical and three non-critical violations.

Critical were temperature-controlled foods not being cold held at the proper temperature (repeat, corrected during inspection); and no air gap or approved backflow prevention device on the plumbing system (repeat).

Non-critical were facility does not have an employee with manager certification in food protection (repeat); handwashing sink water below 100°F (repeat); and non-food contact surfaces of equipment are unclean (repeat).

Obe’s Country Store, 19963 Otsego Pike, Bowling Green, had three critical and three non-critical violations.

Critical were employees are not informed in a verifiable manner of their responsibility to report information about their health; temperature-controlled foods not being hot held at the proper temperature (corrected during inspection); and temperature-controlled foods not being cold held at the proper temperature.

Non-critical were inadequate equipment available for cooling, heating or holding food; equipment components are not intact, tight or properly adjusted; and non-food contact surface(s) not cleaned at the required frequency.

The following inspections were done Oct. 26.

Porky’s Pizza Trof, 13165 Main St. P.O. Box 64, Weston, had two non-critical offenses, which were equipment components are not intact, tight or properly adjusted; and non-food contact surface(s) not cleaned at the required frequency (repeat).

Burger King, 1570 E. Wooster St., Bowling Green, had one critical and one non-critical offense.

Critical was improper use of time as a public health control (four hours). It was corrected during inspection.

Non-critical was non-food contact surfaces of equipment are unclean (repeat).

Subway, 3310 Woodville Road, Northwood, had one critical offense, which was temperature-controlled foods not being cold held at the proper temperature. It was corrected during inspection.

Burger King, 10796 Fremont Pike, Perrysburg, had one non-critical offense, which was facility floor not maintained clean (repeat).

Weston Marathon, 13170 Mill St., had three non-critical offenses, which were equipment components are not intact, tight or properly adjusted (repeat); non-food contact surface(s) not cleaned at the required frequency (repeat); and unnecessary or nonfunctional items and/or litter on premises.

Redneck Willy’s, 13217 Main St., Weston, had two non-critical offenses, which were equipment components are not intact, tight or properly adjusted (repeat); and physical facilities not maintained in good repair.

The following inspections were done Oct. 27.

During a follow-up inspection, Fricker’s, 27390 Helen Drive, Perrysburg, had eight non-critical offenses, which were no soap at handwashing sink(s); food employee(s) not wearing a hair restraint; food employees wearing jewelry on arms or hands during food preparation; cutting blocks or boards cannot be effectively cleaned and sanitized; non-food contact surfaces of equipment are unclean; cooking equipment or pan surfaces unclean; physical facilities not maintained in good repair; and facility not maintained clean.

Speedway, 26020 N. Dixie Hwy., Perrysburg, had four critical and one non-critical violation.

Critical were employees are not informed in a verifiable manner of their responsibility to report information about their health; chlorine sanitizing solution at incorrect temperature and/or concentration (corrected during inspection); refrigerated, ready-to-eat, temperature-controlled foods not properly date marked (corrected during inspection); and warewashing machine does not automatically dispense detergent sanitizer and/or does not have a visual means to ensure detergent and sanitizer is automatically dispensed.

Non-critical was facility not maintained clean (repeat).

Arturo’s Pizza Kitchen, 2507 Oregon Road, Northwood, had two non-critical offenses, which were food employee(s) not wearing a hair restraint (repeat); and floors, walls, and/or ceilings not smooth and easily cleanable (repeat).

Comfort Suites, 27450 Helen Drive, Perrysburg, had one critical and two non-critical offenses.

Critical was temperature-controlled foods not being cold held at the proper temperature. It was corrected during inspection.

Non-critical was packaged foods improperly stored or displayed in direct contact with ice and/or water; and equipment not approved by a recognized testing agency (repeat).

Chipotle Mexican Grill, 10711 Fremont Pike, Perrysburg, had one critical offense, which was temperature-controlled foods not being hot held at the proper temperature (repeat, corrected during inspection).

Odd Fodder, 26520 N. Dixie Hwy., Perrysburg, had one critical and two non-critical offenses.

Critical was temperature-controlled foods not being cold held at the proper temperature. It was corrected during inspection.

Non-critical were improper storage of food items; and non-food contact surface(s) not cleaned at the required frequency.

What the Fried Rice, 2509 Oregon Road, Northwood, had one critical and two non-critical offenses.

Critical was handwashing sink is being used for purposes other than handwashing.

Non-critical were non-food contact surface(s) not cleaned at the required frequency; and facility not maintained clean.

S&G, 25570 Dixie Hwy., Perrysburg, had three critical and six non-critical violations.

Critical were employees are not informed in a verifiable manner of their responsibility to report information about their health; equipment and utensils contacting temperature-controlled food not cleaned every four hours (corrected during inspection); and temperature-controlled foods not being hot held at the proper temperature (repeat, corrected during inspection).

Non-critical were no written procedures for responding to vomiting or diarrheal events could be verified at time of inspection (corrected during inspection); food thermometer not readily accessible (repeat); bulk food for customer self-service not properly labeled (repeat); food service items on floor in back storage room; physical facilities not maintained in good repair; and retail food establishment does not comply with Level One Certification requirements.

The following inspections were done Oct. 28.

During a follow-up inspection, Panera Bread, 10981 Fremont Pike, Perrysburg, had five non-critical violations, which were improper storage of food items; food employees wearing jewelry on arms or hands during food preparation; improper use and/or maintenance of wiping cloths; non-food contact surfaces of equipment are unclean; and facility walls and ceiling not maintained clean.

During a follow-up inspection, Arby’s Perrysburg, 26530 N. Dixie Hwy., Perrysburg, had one critical and three non-critical offenses.

Critical was no air gap or approved backflow prevention device on the plumbing system.

Non-critical were inadequate equipment available for cooling, heating or holding food; no sanitizer test kit available; and non-food contact surfaces of equipment are unclean.

Zero violations were found at Rossford PK-5 School, 28500 Lime City Road, Rossford; Rossford 6-12 School, 701 Superior St., Rossford; Doc’s Rib Cage, 18625 Main St., Tontogany; Perrysburg Junior High School, 550 E. South Boundary, Perrysburg; Jimmy John’s, 10081 Fremont Pike, Perrysburg; Penta Career Center (MME), 9301 Buck Road, Perrysburg; and Meijer Gas Station #116, 2801 Curtice Road, Northwood.
