Notice to the Public It was discovered, on March 22,2013, that on March
19, 2013, radiology films being prepared fordestruction by recycling were stolen from a records storage room
at WoodCounty Hospital. These were films that had been taken prior to March19, 2013 and had already been
digitally converted into the hospitalelectronic records system. It is our belief that the films were stolenin
order to recover the raw material, silver, from the film. The onlyway to obtain the silver would involve
destruction of the films and, therefore, it is unlikely that the films still exist. Depending on
theexamination, the film contained patient name, medical record number, date of examination and in some cases
date of birth.Films ofapproximately 2000 to 2500 individuals were stolen.Wood CountyHospital is working
collaboratively with the Bowling Green PoliceDepartment in solving this crime.Wood County Hospital
internally hasimplemented corrective actions to increase film storage security inresponse to the theft. Wood
County Hospital is not able to identifywhich patients’ films were stolen and is, therefore, unable to
contactpatients directly to notify them.It is intended that this publicnotice serve as notice to
patients.Individuals who believe they mayhave been affected by this theft may contact the following toll
freenumber: 855-322-6427 if they have questions. May 10, ’13

May 10, ’13
