The following inspections were done April 25-

Speedway, 26020 N. Dixie Hwy., Perrysburg, had two critical and two non-critical offenses.

Critical were temperature-controlled foods not being cold held at the proper temperature; and refrigerated, ready-to-eat, temperature-controlled foods not properly date marked (repeat). Both were corrected during inspection.

Non-critical were mops dried improperly; and facility not maintained clean (repeat).

Wendy’s Old Fashioned Hamburgers, 1094 S. Main St., Bowling Green, had one critical and two non-critical offenses.

Critical was temperature-controlled foods not being cold held at the proper temperature. It was corrected during inspection.

Non-critical were non-food contact surfaces of equipment are unclean (repeat); and facility not maintained clean.

During a follow-up inspection, Hershey’s Ice Cream DBA Nedley’s Ice Cream & Coffee Cafe’, 200 E. South Boundary St., Perrysburg, had three non-critical offenses, which were equipment not approved by a recognized testing agency; non-food contact surfaces of equipment are unclean (corrected during inspection); and facility flooring not maintained clean (repeat, corrected during inspection).

Buffalo Wild Wings, 1550 E. Wooster St., Bowling Green, had two critical and one non-critical offense.

Critical were food not properly protected from contamination by separation, packaging, and segregation; and temperature-controlled foods not being cold held at the proper temperature. Both were corrected during inspection.

Non-critical was non-food contact surfaces of equipment are unclean (repeat).

Burger King, 1181 S. Main St., Bowling Green, had one critical and two non-critical offenses.

Critical ready-to-eat, temperature-controlled food not properly discarded when required. It was corrected during inspection.

Non-critical were non-food contact surfaces of equipment are unclean (repeat); and floors, walls, and/or ceilings not smooth and easily cleanable.

Taco Bell, 25782 N. Dixie Hwy., Perrysburg, had one critical and one non-critical offense.

Critical was Equipment food-contact surfaces or utensils are unclean (repeat, corrected during inspection).

Non-critical was equipment and utensils are not being air dried prior to stacking (repeat).

Pub 1910, 101 Bowen-Thompson Student Union – BGSU, Bowling Green, had three critical offenses, which were temperature-controlled foods not being hot held at the proper temperature; temperature-controlled foods not being cold held at the proper temperature; and improper storage of poisonous or toxic materials. All were corrected during inspection.

The following inspections were done April 26.

Wood County Committee on Aging-Perrysburg, 140 W. Indiana Ave., had one critical offense, which was temperature-controlled foods not being cold held at the proper temperature. It was corrected during inspection.

Circle K, 1602 E. Wooster St., Bowling Green, had three critical and three non-critical violations.

Critical were employees are not informed in a verifiable manner of their responsibility to report information about their health (repeat); food not properly protected from contamination by separation, packaging, and segregation (corrected during inspection); and missing and/or incomplete allergen declaration(s) (repeat).

Non-critical were gloves were stored in cabinet under handwashing sink in register food prep area (repeat); non-food contact surfaces of equipment are unclean; and physical facilities not maintained in good repair (repeat).

St. Clare Commons, 12469 Five Point Road, Perrysburg, had two critical and three non-critical offenses.

Critical were temperature-controlled foods not being hot held at the proper temperature; and temperature-controlled foods not being cold held at the proper temperature (repeat). Both were corrected during inspection.

Non-critical were unnecessary medicines and/or improper labeling/storing of medicines (corrected during inspection); equipment and utensils are not being air dried; and non-food contact surfaces of equipment are unclean (repeat).

GLCAP Perrysburg-Rossford Early Childhood Center, 28744 Simmons Road, had one critical offense, which was receiving refrigerated, frozen or hot food at improper temperatures or with evidence of temperature abuse.

Home 2 Suites by Hilton, 1630 E. Wooster St., Bowling Green, had two critical and four non-critical violations.

Critical were refrigerated, ready-to-eat, temperature-controlled foods not properly date marked (corrected during inspection); and improper use of time as a public health control (four hours).

Non-critical were improper storage of food items (repeat); raw fruit and/or vegetables not washed or washed improperly (repeat); equipment not approved by a recognized testing agency; and food service operation did not have a person in charge per shift with the person in charge certification in food protection (repeat).

Peach Cobbler Factory, 10584 Fremont Pike, Perrysburg, had three non-critical offenses, which were food employee(s) not wearing a hair restraint; dressing rooms, designated areas or lockers not being properly used; and food service operation did not have a person in charge per shift with the person in charge certification in food protection.

The following inspections were done April 27.

During a follow-up inspection, O-Deer Diner, 416 Louisiana Ave., Perrysburg, had two non-critical offenses, which were non-food contact surfaces not easily cleanable; and food service operation did not have a person in charge per shift with the person in charge certification in food protection.

During a follow-up inspection, Pizza Hut, 1099 S. Main St., Bowling Green, had one critical and five non-critical offenses.

Critical was chlorine sanitizing solution at incorrect temperature and/or concentration (repeat).

Non-critical were equipment components are not intact, tight or properly adjusted; non-food contact surfaces of equipment are unclean; physical facilities not maintained in good repair (repeat); facility not maintained clean; and floors, walls, and/or ceilings not smooth and easily cleanable (repeat).

During a follow-up inspection, Bob Evans Restaurants, 10770 Fremont Pike, Perrysburg, had two critical and eight non-critical violations.

Critical were food employee(s) did not wash hands when required; and temperature-controlled foods not being cold held at the proper temperature.

Non-critical were employee eating, drinking, or using tobacco in non-designated area; improper storage of food items (repeat, corrected during inspection); non-food contact surfaces not easily cleanable (repeat); warewashing equipment and/or components not cleaned at required frequency (repeat); non-food contact surfaces of equipment are unclean (repeat); cooking equipment or pan surfaces unclean (repeat); physical facilities not maintained in good repair (repeat); and facility not maintained clean (repeat).

RAM-Z’s, 269 Brown St., Jerry City, had two critical offenses, which were handwashing sink not accessible; and temperature-controlled foods not being cold held at the proper temperature. Both were corrected during inspection.

The following inspections were done April 28.

Days Inn/Super-8, 10667 Fremont Pike, Perrysburg, had two critical and three non-critical offenses.

Critical were equipment food-contact surfaces or utensils are unclean (corrected during inspection); and temperature-controlled foods not being cold held at the proper temperature (repeat).

Non-critical were carpet installed in unapproved areas; food service operation did not submit facility layout and equipment specifications; and food service operation did not have a person in charge per shift with the person in charge certification in food protection.

The Oaks Dining Center, BGSU, Bowling Green, had three critical and three non-critical violations.

Critical were temperature-controlled foods not being cold held at the proper temperature (pizza cooler unable to maintain proper temperature); temperature-controlled foods not being cold held at the proper temperature (rice condiment cooler holding food at an improper temperature); and temperature-controlled foods not being cold held at the proper temperature (prep top cooler holding burger toppings and condiments at improper temperatures).

Non-critical were inadequate equipment available for cooling, heating or holding food; inadequate equipment available for cooling, heating or holding food; and nonfood-contact surfaces not easily cleanable. All three were repeat offenses.

During a follow-up inspection, Kabob It, 132 E. Wooster St., Bowling Green, had two non-critical offenses, which were non-food contact surfaces not easily cleanable (repeat); and non-food contact surfaces of equipment are unclean.

City Egg, 121 S. Main St., Bowling Green, had one non-critical offense, which was improper use and/or maintenance of wiping cloths.
