Health department conducts inspections

A Perrysburg eatery had 17 total violations, including dirty hands and food not being kept cold, during a recent health inspection.

The Wood County Health Department inspected Golden House Restaurant, 130 E. South Boundary St., on April 20 and found seven critical and 10 non-critical violations.

Critical were person in charge did not ensure employees are properly maintaining temperatures of temperature-controlled foods during hot and cold storage; food employee(s) hands and/or arms not clean (observed dirty nails and fingers on male owner, as he was handling and touching food and equipment); food not properly protected from contamination by separation, packaging, and segregation (repeat, corrected during inspection); food in contact with an unclean surface or linens (corrected during inspection); food in contact with an unclean surface or linens (observed male owner dragging net bag of cabbage heads across floor to prep table to be cut); temperature-controlled foods not being cold held at the proper temperature (observed cooked chicken, crab rangoon, egg rolls and chicken breasts in tabletop reach in cooler; and cooked steak, cooked chicken, cooked broccoli on tabletop all at improper temperatures; products were discarded (repeat, corrected during inspection); and refrigerated, ready-to-eat, temperature-controlled foods not properly date marked (corrected during inspection).

Non-critical were inadequate equipment available for cooling, heating or holding food; outer opening not protected; raw fruit and/or vegetables not washed or washed improperly; improper storage of single-service and single-use articles; warewashing equipment and/or components not cleaned at required frequency; cooking equipment or pan surfaces unclean; non-food contact surfaces of equipment are unclean (repeat); facility not maintained clean; physical facilities not maintained in good repair (repeat); and unnecessary or nonfunctional items and/or litter on premises (repeat).

The following inspections were done April 18.

During a complaint inspection, Scramblers, 25690 Dixie Hwy., Perrysburg, had four critical and six non-critical violations.

The department was told that on April 12 around 11:30 p.m., several customers’ water glasses smelled like fish. Not the water, but the actual glass it was in.

The person in charge said they were aware of the complaint. The high temperature dish machine that the cups are washed in was reaching the proper temperature at time of inspection.

Critical were handwashing sink by cookline observed blocked by trash can and not readily accessible (repeat); food not properly protected from contamination by separation, packaging, and segregation; temperature-controlled foods not being cold held at the proper temperature (repeat); and improper storage of poisonous or toxic materials (repeat). All were corrected during inspection.

Non-critical were inadequate equipment available for cooling, heating or holding food; improper storage of food items (repeat); improper use and/or maintenance of wiping cloths; non-food contact surfaces not easily cleanable (repeat); non-food contact surfaces of equipment are unclean (repeat); and facility not maintained clean (repeat).

Bowling Green Hospitality Group Inc., 2150 E. Wooster St., Bowling Green, had one critical and five non-critical violations.

Critical was employees are not informed in a verifiable manner of their responsibility to report information about their health (repeat, corrected during inspection).

Non-critical were facility does not have an employee with manager certification in food protection (repeat); no written procedures for time as a public health control (repeat, corrected during inspection); food employee(s) not wearing a hair restraint (repeat); raw fruit and/or vegetables not washed or washed improperly (repeat, corrected during inspection); and non-food contact surfaces of equipment are unclean.

During a complaint inspection, Moe’s Place, 620 Dixie Hwy., Rossford, had one non-critical offense, which was no soap at handwashing sink(s). It was corrected during inspection.

The complaint was received April 18 and upon inspection, all restrooms at the facility were in good working condition. The only issue found was related to the empty soap dispenser at the handwashing sink in the kitchen area.

Baymont by Wyndham, 27441 Helen Drive, had three critical and three non-critical violations.

Critical were handwashing sink is being used for purposes other than handwashing (corrected during inspection); quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness (repeat, corrected during inspection); and temperature-controlled foods not being cold held at the proper temperature (repeat).

Non-critical was facility does not have an employee with manager certification in food protection; packaged foods improperly stored or displayed in direct contact with ice and/or water (repeat); and equipment not approved by a recognized testing agency.

During a complaint inspection, Pertoria Inc. DBA Wendy’s, 26630 Dixie Hwy., Perrysburg, had one critical offense, which was temperature-controlled foods not being cold held at the proper temperature. It was corrected during inspection.

A complainant reportedly observed staff preparing food without gloves. The complainant and family consumed Dave’s single combo, fries, cheeseburger and nuggets on April 9 and became ill shortly after. Proper handling of foods was observed at the time of inspection.

McDonald’s Corporation, 10163 Fremont Pike, Perrysburg, had two critical and 10 non-critical violations.

Critical were chlorine sanitizing solution at incorrect temperature and/or concentration; and temperature-controlled foods not being hot held at the proper temperature. Both were corrected during inspection.

Non-critical were handwashing sink water below 100°F; inadequate equipment available for cooling, heating or holding food; improper storage of food items; food employees wearing jewelry on arms or hands during food preparation; food employee(s) not wearing a hair restraint (corrected during inspection); non-food contact surfaces not easily cleanable; equipment components are not intact, tight or properly adjusted; warewashing equipment and/or components not cleaned at required frequency; non-food contact surfaces of equipment are unclean (repeat); and plumbing system not properly maintained or repaired.

Subway, 10392 Fremont Pike, Perrysburg, had one critical and four non-critical offenses.

Critical was no air gap or approved backflow prevention device on the plumbing system (repeat).

Non-critical were facility does not have an employee with manager certification in food protection (repeat); handwashing sink water below 100°F (repeat); utensils and equipment contacting non-temperature-controlled foods not cleaned at required frequency; and food service operation did not have a person in charge per shift with the person in charge certification in food protection.

Kim’s Kakes LLC, 480 Dixie Hwy. Unit B, Rossford, had two non-critical offenses, which were employee eating, drinking, or using tobacco in a non-designated area; and working food containers not properly labeled.

The following inspections were conducted April 20.

Perrysburg Senior High School, 13385 Roachton Road, had one critical offense, which was temperature-controlled foods not being cold held at the proper temperature (repeat, corrected during inspection).

Speedway, 1650 E. Wooster St., Bowling Green, had two critical and nine non-critical violations.

Critical were handwashing sink not accessible (corrected during inspection); and improper storage of poisonous or toxic materials.

Non-critical were employee eating, drinking, or using tobacco in non-designated area; working food containers not properly labeled; outer opening not protected (repeat); single-use condiments improperly stored; dispensing tube on bulk milk container not cut properly; non-food contact surfaces not easily cleanable; waste receptacles or handling units not cleaned and maintained; facility not maintained clean (repeat); and mops dried improperly.

Rite Aid, 1175 Louisiana Ave., Perrysburg, had one critical and two non-critical offenses.

Critical was temperature-controlled foods not being cold held at the proper temperature. It was corrected during inspection.

Non-critical were inadequate equipment available for cooling, heating or holding food; and packaged foods improperly stored or displayed in direct contact with ice and/or water.

During a follow-up inspection, North Baltimore Convenience Station, 101 S. Main St., had two critical and one non-critical offense.

Critical were no air gap or approved backflow prevention device on the plumbing system; and insufficient air gap between the flood rim and the water supply inlet.

Non-critical was physical facilities not maintained in good repair.

Travel Store, 12906 Deshler Road, North Baltimore, had two critical and two non-critical offenses.

Critical were refrigerated, ready-to-eat, temperature-controlled foods not properly date marked (repeat); and ready-to-eat, temperature-controlled food not properly discarded when required. Both were corrected during inspection.

Non-critical were non-food contact surfaces of equipment are unclean; and facility not maintained clean. Both were corrected during inspection.

South Side Six, 737 S. Main St., Bowling Green, had six non-critical offenses, which were food packaged in the facility not labeled; label on a food packaged in-house did not contain all required information (repeat); improper storage of food items (corrected during inspection); non-durable equipment observed (corrected during inspection); no drain plugs in waste receptacle; and physical facilities not maintained in good repair.

Kreischer Dining Hall, 1416 Ridge St., Bowling Green, had four non-critical offenses, which were non-food contact surfaces not easily cleanable (repeat); non-food contact surfaces of equipment are unclean (repeat); unnecessary or nonfunctional items and/or litter on premises; and physical facilities not maintained in good repair.

Carillon Place Dining Center, Bowling Green State University, Bowling Green, had four critical and two non-critical offenses.

Critical were temperature-controlled foods not being hot held at the proper temperature (several areas on the hot holding line that were not holding food at proper temperature); temperature-controlled foods not being hot held at the proper temperature (food items in the Choolah steam table not holding proper temperature); temperature-controlled foods not being cold held at the proper temperature (pasta area prep top cooler at an improper temperature; and temperature-controlled foods not being cold held at the proper temperature (sour cream in a bottle stored in a cold hold line was at an improper temperature).

Non-critical were inadequate equipment available for cooling, heating or holding food; and equipment components are not intact, tight or properly adjusted.

Or’s Sunoco LLC, 118 N. Findlay St., Haskins, had one critical violation, which was food not properly protected from contamination by separation, packaging, and segregation. It was corrected during inspection.

Rick’s Carry Out Store, 10015 S. Dixie Hwy., Portage, had two non-critical offenses, which were floors, walls, and/or ceilings not smooth and easily cleanable; and floor and wall junctures not properly coved or closed and/or floor drains not provided.

Market at Kreischer, 1416 Ridge St., Bowling Green, had two non-critical offenses, which were label on a food packaged in-house did not contain all required information; and non-food contact surfaces not easily cleanable (repeat).

BGSU – Maurer Center, Coffee/Café, 819 E. Wooster St., Bowling Green, had one non-critical offense, which was label on a food packaged in-house did not contain all required information.

Dollar General, 1107 East Bowling Green Road, Bradner, had two critical offenses, which were improper storage of poisonous or toxic materials; and food in a retail food establishment must be embargoed due to adulteration, misbranding, or baby food or formula is expired. Both were corrected during inspection.

The following inspections were done April 21.

Falcon Nest Food Court, Bowen-Thompson Student Union, Bowling Green, had four critical and five non-critical violations.

Critical were food employee(s) did not wash hands when required (corrected during inspection); equipment food-contact surfaces or utensils are unclean; temperature-controlled foods not being cold held at the proper temperature (poke bowls, sushi, sandwiches, and juice at improper temperatures in the display case) (corrected during inspection); and temperature-controlled foods not being cold held at the proper temperature (chicken, tomatoes, turkey, and lettuce in the cold well at improper temperature) (corrected during inspection).

Non-critical were improper method for cooling temperature-controlled foods; inadequate equipment available for cooling, heating or holding food (Houston sub area cold well holding at improper temperatures); inadequate equipment available for cooling, heating or holding food (sushi display cooler holding some foods at improper temperature); non-food contact surfaces not easily cleanable (repeat); and non-food contact surfaces of equipment are unclean.

Zero violations were found at Frank Elementary School, 401 W. South Boundary St., Perrysburg; Jonathon Khoi Nail Spa – Rossford, 9822 Fremont Pike; Amazon-PCW1-4th Floor Back Breakroom-Market C, 27400 Crossroads Pkwy., Rossford; Frank’s Fries #3, 601 E. Crocker St., Bradner; North Baltimore Middle School/High School, 2012 Tiger Drive; Hospice of Northwest Ohio, 30000 E. River Road, Perrysburg; and Powell Elementary School, 500 N. Main St., North Baltimore.
