Health department conducts inspections

The Wood County Health Department conducted inspections in Perrysburg, Bowling Green, Northwood and Rossford during the second week of April.

The following inspections were done April 10.

Dollar General Store, 1125 S. Main St., Bowling Green, had one non-critical offense, which was facility not maintained clean (repeat).

Burger King, 10796 Fremont Pike, Perrysburg, had two critical and one non-critical offense.

Critical were temperature-controlled foods not being hot held at the proper temperature; and improper use of time as a public health control (four hours). Both were corrected during inspection.

Non-critical was non-food contact surfaces of equipment are unclean.

The following inspections were done April 11.

Campus Market, 780 S. College Drive, Bowling Green, had one non-critical offense, which was equipment components are not intact, tight or properly adjusted (repeat).

Mike’s Party Mart, 834 S. Main St., Bowling Green, had two non-critical offenses, which were service sink or curbed cleaning facility not provided and/or conveniently located; and Physical facilities not maintained in good repair.

BP Woodville, 3808 Woodville Road, Northwood, had two critical and two non-critical offenses.

Critical were conditional employee not properly excluded or restricted; and improper use of a poisonous or toxic substance (corrected during inspection).

Non-critical were no written procedures for responding to vomiting or diarrheal events; and retail food establishment does not comply with Level One Certification requirements.

The following inspections were done April 12.

YMCA of Greater Toledo/Owens Community College Childcare Center, 30335 Oregon Road, Perrysburg, had one non-critical offense, which was physical facilities not maintained in good repair.

Comfort Suites, 27450 Helen Drive, Perrysburg, had two critical and two non-critical offenses.

Critical were temperature-controlled foods not being cold held at the proper temperature (repeat); and no air gap or approved backflow prevention device on the plumbing system.

Non-critical were food stored in prohibited area; and equipment not approved by a recognized testing agency (repeat).

Market at Carillon Place Carillon Place Dining Center – BGSU, Bowling Green, had two non-critical offenses, which were non-food contact surfaces not easily cleanable (repeat); and retail food establishment constructed, altered, and/or equipment added without prior approval from the licensor.

Market at Offenhauer, 522 W. Thurstin Ave., Bowling Green, had three non-critical offenses, which were improper storage of food items; equipment components are not intact, tight or properly adjusted; and non-food contact surfaces not easily cleanable (repeat).

During a follow-up inspection, Sweetfish – Bubble Tea Poke Bowl, 26597 N. Dixie Hwy., Perrysburg, had five non-critical violations, which were food thermometer not readily accessible; non-food contact surfaces not easily cleanable; equipment not approved by a recognized testing agency; floors, walls, and/or ceilings not smooth and easily cleanable; and dressing rooms or lockers not being properly used.

The following inspections were done April 13.

Friendship Food Store, 24010 Front St., Grand Rapids, had two critical and three non-critical violations.

Critical were employees are not informed in a verifiable manner of their responsibility to report information about their health (repeat); and improper storage of poisonous or toxic materials (repeat).

Non-critical were no written procedures for responding to vomiting or diarrheal events; improper use or placement of insect control devices (corrected during inspection); and waste receptacles not covered properly.

Dollar General, 18300 Pemberville Road, Pemberville, had three non-critical offenses, which were employee eating, drinking, or using tobacco in non-designated area; waste receptacles for refuse or recyclables stored so they are accessible to insects and/or rodents; and mops dried improperly.

Pemberville Carry Out, 450 Front St., had one non-critical offense, which was hot and cold holding equipment thermometer was missing, located incorrectly, or not easily readable.

Pumps of Peace DBA Way Makers, 24125 Front St., Grand Rapids, had one non-critical offense, which was equipment not approved by a recognized testing agency.

The following inspections were done April 14.

During a complaint inspection, Bar Louie, 4105 Levis Commons Blvd., Perrysburg, had two non-critical offenses, which were non-food contact surfaces of equipment are unclean; and physical facilities not maintained in good repair (repeat).

The department received a complaint on April 14 that three people had eaten pasta, trio dip and southern mac and cheese on April 12 and became ill that evening. The person in charge reported the facility had not had any employees who have reported ill. All food items observed during inspection were being held and cooked at proper temperatures.

Panda Express, 9990 Olde U.S. 20, Rossford, had one critical offense, which was temperature-controlled foods not being hot held at the proper temperature (repeat, corrected during inspection).

During a follow-up inspection, Get Healthy Nutrition, 126 W. South Boundary St., Perrysburg, had one critical and five non-critical violations.

Critical was raw shell eggs not maintained at 45° F or below (repeat, corrected during inspection).

Non-critical were handwashing sink water below 100°F; inadequate equipment available for cooling, heating or holding food (repeat); improper storage of food items; food employee(s) not wearing a hair restraint; and equipment not approved by a recognized testing agency.

Jimmy John’s, 10081 Fremont Pike, Perrysburg, had one critical and seven non-critical violations.

Critical was improper storage of poisonous or toxic materials. It was corrected during inspection.

Non-critical were facility does not have an employee with manager certification in food protection; outer opening not protected; food employee(s) not wearing a hair restraint (corrected during inspection); cutting blocks or boards cannot be effectively cleaned and sanitized; physical facilities not maintained in good repair; no protective shielding on lights; and food service operation did not have a person in charge per shift with the person in charge certification in food protection.

Zero violations were found at Aldi Inc., 9806 S. Compass Drive, Rossford; Five Below Inc., 1111 S. Main St., Bowling Green; Doggs Ice Cream & Bites, 640 N. Dixie Hwy., Rossford; Rite Aid, 722-740 S. Main St., Bowling Green; Big Lots LLC, 818 S. Main St., Bowling Green; Chilly Treats, 4568 Woodville Road, Northwood; McDonald’s, 2700 Woodville Road, Northwood; Market At Jerome Library, Jerome Library – BGSU, Bowling Green; Wood County Committee on Aging – Northeast Center, 705 N. Main St., Walbridge; Rossford PK-5 School, 28500 Lime City Road; Wellaroma, 136 E. Third St., Perrysburg; NAMSA – Tracy Road 2nd Floor, 6750 Wales Road, Northwood; Wood County Committee on Aging – Pemberville, 200 Cedar St.; WCESC Pemberville Preschool, 120 College Ave.; Wood County Committee on Aging — Grand Rapids, 23019 Kellogg Road; Walgreens, 10003 Fremont Pike, Perrysburg; and Fed Ex-Ground, 2490 Woodstream Drive, Bowling Green.
