Health dept. conducts inspections

The Wood County Health Department concentrated in Perrysburg, Rossford and Bowling Green for its inspections during end of September and the first week of this month.

The following inspections were done Sept. 29.

Little Caesars Pizza, 154 W. South Boundary St., Perrysburg, had two critical and four non-critical violations.

Critical were written procedures and plans are not maintained and implemented (repeat); and temperature-controlled foods not being cold held at the proper temperature. Both were corrected during inspection.

Non-critical were working food containers not properly labeled; food employees wearing jewelry on arms or hands during food preparation; food employee(s) not wearing a hair restraint (repeat); and non-food contact surfaces of equipment are unclean (repeat).

Domino’s Pizza, 154 E. South Boundary St., Perrysburg, had two critical and five non-critical violations.

Critical were employees are not informed in a verifiable manner of their responsibility to report information about their health; and working containers of poisonous or toxic materials not properly labeled (corrected during inspection).

Non-critical were facility does not have an employee with manager certification in food protection; outer opening not protected; food employee(s) not wearing a hair restraint; fixed equipment not properly sealed or spaced for cleaning; and retail food establishment does not comply with Level One certification requirements.

City Tap and the Attic, 110 N. Main St., Bowling Green, had two critical and six non-critical violations.

Critical were handwashing sink not accessible; and chlorine sanitizing solution at incorrect temperature and/or concentration (repeat).

Non-critical were facility does not have an employee with manager certification in food protection (repeat); utensils and/or equipment not properly rinsed; no sanitizer test kit available; non-food contact surfaces of equipment are unclean; facility not maintained clean (repeat); and physical facilities not maintained in good repair (repeat).

Holiday Inn, 27355 Carronade Drive, Perrysburg, had two critical and six non-critical violations.

Critical were raw shell eggs not maintained at 45° F or below; and temperature-controlled foods not being cold held at the proper temperature. Both were corrected during inspection.

Non-critical were improper storage of food items; non-food contact surfaces not easily cleanable; non-food contact surfaces of equipment are unclean (repeat); cooking equipment or pan surfaces unclean; physical facilities not maintained in good repair; and food service operation did not have a person in charge per shift with the person in charge certification in food protection (corrected during inspection).

Perrysburg Healthcare and Rehabilitation Center, 28546 Starbright Blvd., had two non-critical offenses, which were non-food contact surfaces of equipment are unclean (repeat); and food service operation did not have a person in charge per shift with the person in charge certification in food protection.

During a complaint inspection, Condado Tacos, 42 LLC, 10584 Fremont Pike Suite 4, Perrysburg, had one critical offense, which was temperature-controlled foods not being hot held at the proper temperature (repeat).

A person reported on Sept. 28 that two out of three individuals fell ill immediately after eating/drinking at the restaurant. The inspector spoke to the person in charge and could not verify the complaint.

The following inspections were done Sept. 30.

Bob Evans Restaurants LLC, 1313 Levis Commons Blvd., Perrysburg, had four critical and six non-critical violations.

Critical were employees are not informed in a verifiable manner of their responsibility to report information about their health; equipment food-contact surfaces or utensils are unclean; raw shell eggs not maintained at 45° F or below (corrected during inspection); and working containers of poisonous or toxic materials not properly labeled (corrected during inspection).

Non-critical were improper storage of food items; food employee(s) not wearing a hair restraint; in-use utensils improperly stored (corrected during inspection); cooking equipment or pan surfaces unclean; non-food contact surfaces of equipment are unclean (repeat); and facility not maintained clean (repeat).

City Old Town Buffet Inc., 1216 N. Main St., Bowling Green, had two critical and three non-critical offenses.

Critical were temperature-controlled foods not being hot held at the proper temperature; and working containers of poisonous or toxic materials not properly labeled (corrected during inspection).

Non-critical were in-use utensils improperly stored (corrected during inspection); equipment components are not intact, tight or properly adjusted (repeat); and physical facilities not maintained in good repair.

During a follow-up inspection, Pisanello’s Pizza, 203 N. Main St., Bowling Green, had one non-critical offense, which was physical facilities not maintained in good repair.

During a follow-up inspection, Uptown-Downtown, 162 N. Main St., Bowling Green, had one critical and six non-critical offenses.

Critical was gnats and flies observed in the facility

Non-critical were equipment components are not intact, tight or properly adjusted; nonfood-contact surfaces not easily cleanable; non-food contact surface(s) not cleaned at the required frequency; plumbing system not properly maintained or repaired; physical facilities not maintained in good repair; and facility not maintained clean.

Carranor Hunt & Polo Club, 502 E. Second St., Perrysburg, had two critical and two non-critical offenses.

Critical were temperature-controlled foods not being hot held at the proper temperature; and temperature-controlled foods not being cold held at the proper temperature.

Non-critical were non-food contact surfaces of equipment are unclean; and cooking equipment or pan surfaces unclean.

The following inspections were done Oct. 3.

Pilot Travel Center, 3484 Libbey Road, Perrysburg, had three non-critical offenses, which were non-food contact surface(s) not cleaned at the required frequency; plumbing system not properly maintained or repaired; and physical facilities not maintained in good repair (repeat).

During a follow-up inspection, Fat Fish Blue Home of the Funny Bone, 6140 Levis Commons Blvd., Perrysburg, had one critical and five non-critical violations.

Critical was employees are not informed in a verifiable manner of their responsibility to report information about their health.

Non-critical were inadequate equipment available for cooling, heating or holding food; non-food contact surfaces of equipment are unclean; physical facilities not maintained in good repair; no protective shielding on lights; and food service operation did not have a person in charge per shift with the person in charge certification in food protection.

Fuel Mart, 3654 Libbey Road, Perrysburg, had one critical and four non-critical offenses.

Critical was improper storage of poisonous or toxic materials (repeat).

Non-critical were temperature-controlled foods not properly thawed (corrected during inspection); single-service and single-use articles not protected from contamination; non-food contact surfaces of equipment are unclean (repeat); and mops dried improperly.

St. Clare Commons, 12469 Five Point Road, Perrysburg, had five critical and six non-critical violations.

Critical were chlorine sanitizing solution at incorrect temperature and/or concentration; equipment food-contact surfaces or utensils are unclean (repeat); improper cooling of temperature-controlled food (corrected during inspection); temperature-controlled foods not being cold held at the proper temperature (repeat, corrected during inspection); and refrigerated, ready-to-eat, temperature-controlled foods not properly date marked (corrected during inspection).

Non-critical were inadequate equipment available for cooling, heating or holding food; improper method for cooling temperature-controlled foods; Improper storage of food items; equipment and utensils are not being air dried (corrected during inspection); non-food contact surfaces not easily cleanable; and non-food contact surfaces of equipment are unclean (repeat).

Hissho Sushi @ Meijer, 10055 Olde U.S. 20, Rossford, had one critical offense, which was ready-to-eat, temperature-controlled food not properly discarded when required. It was corrected during inspection.

Total Sports Grill, 10020 S Compass Drive, Rossford, had two non-critical offenses, which were facility does not have an employee with manager certification in food protection; and floors, walls, and/or ceilings not smooth and easily cleanable (repeat).

Chayhana TM LLC, 3565 Libbey Road, Perrysburg, had three critical and three non-critical violations.

Critical were employees are not informed in a verifiable manner of their responsibility to report information about their health (repeat); food not properly protected from contamination by separation, packaging, and segregation (corrected during inspection); and improper cooling of temperature-controlled food (corrected during inspection).

Non-critical were no towels or drying device at the handwashing sink(s) (corrected during inspection); temperature-controlled foods not properly thawed (repeat, corrected during inspection); and improper storage of food items.

Zero violations were found at Northwest Community Corrections Center, 1740 E. Gypsy Lane Road, Bowling Green; Hot Dawg Hut, 14103 Hannah Road, Bowling Green; Costco Wholesale, 26400 N. Dixie Hwy., Perrysburg; Walmart, 131 W. Gypsy Lane Road, Bowling Green; SolFood Collective LLC, 119 W. Second St., Perrysburg; and Great Scot, 13710 Deshler Road, North Baltimore.
