There are many senior citizens who are doing extraordinary things in their communities.
If you know a senior citizen volunteer who enhances the lives of others, an opportunity is available to
nominate them for the annual Outstanding Senior Volunteer Statewide Awards sponsored by Medical Mutual
of Ohio.
This event will honor senior citizens throughout Ohio age 65 and over, who volunteer their time in a
selfless, caring manner and are committed to enhancing the quality of life for people in their
communities.
The top four award winners, "platinum, gold, silver and bronze," as well as honorable mention
winners will receive plaques at an awards luncheon, to be held Oct. 8 at Landerhaven, Mayfield Heights.

To qualify, seniors must be nominated by a community organization where they volunteer.
Seniors previously nominated in the regional awards division can be nominated again for the statewide
competition. Community organizations may send a request for a nomination entry form to: Traci Haynes MZ:
01-5A-6920, Medical Mutual, 2060 East 9th St., Cleveland, OH 44115-1355 or fax (216) 687-6164.
Nominations may also be submitted online by logging on to www.Medmutual.com and, by clicking on the
"Community" tab, you will be taken directly to a link for the online Outstanding Senior
Volunteer nomination page. Fill out the online form and click "submit."
Entries must be returned by Aug. 7.
The award recipients and their nominators will attend the luncheon for free. It is not necessary to be a
Medical Mutual of Ohio cardholder to be nominated or participate.
