SEALED BIDS for the furnishing of the necessary materials and construction of the
2009 STREETS AND
SEWER IMPROVEMENTS
MILLBURY, OHIO
will be received by the Village of Millbury, Ohio at the office of Engineer — Poggemeyer Design Group,
Inc., 1168 North Main Street, Bowling Green, Ohio 43402 until
11:30 a.m. (Local Time)
Friday, June 19, 2009
and at that time and place will be publicly opened and read aloud.
The scope of work consists of miscellaneous street resurfacing and storm improvement projects in the
Village including two (2) resurfacing projects and three (3) storm projects. Village reserves the right
to award any or all projects.
The contract documents, including plans and specifications, are on file at the office of the Village of
Millbury, Ohio and the Engineer — Poggemeyer Design Group, Inc. (PDG). A set may be obtained from PDG.
A non-refundable deposit in the amount of $35.00 will be required for each set of plans and
specifications, check must be made payable to Poggemeyer Design Group, Inc.
The Engineer for the Project is Poggemeyer Design Group, Inc., 1168 North Main Street, Bowling Green,
Ohio 43402.
All bids must be signed and submitted on the blanks which are bound in the specification booklet. Bid
must state the unit prices in the blanks provided and be enclosed in a sealed envelope marked —
2009 Streets and Sewer Improvements, Millbury, Ohio — and addressed to
the Engineer — Poggemeyer Design Group, Inc., 1168 North Main Street, Bowling Green, Ohio 43402.
The bid guaranty may be of two forms:
1. A Bid Guaranty and Contract Bond using the form in the Contract
Documents. (The amount of the bid does NOT have to appear on this form.)
2. A certified check, cashier’s check or letter of credit in favor of the
Village of Millbury, Ohio, in the amount of 10% of the bid. If the contract is awarded, a Contract Bond
will be required, which is a 100% payment and performance bond.
After the award of the contract let by competitive bid and prior to the time the contract is entered
into, bidders shall submit the affidavit required under the Ohio Revised Code, Section 5719.042 that the
bidder was not charged with any delinquent personal property taxes in Wood County, Ohio.
The successful bidder will be required to pay not less than the minimum wage rates established by the
Department of Industrial Relations of the State of Ohio.
The Village of Millbury, Ohio reserves the right to reject any and all bids and to waive any irregularity
in any bid and to determine the lowest and best bidder.
No bidder may withdraw his bid for a period of 60 days after the scheduled closing time for the receipt
of bids.
The estimate for the cost of construction is $93,000.

NOTICE OF PUBLIC HEARING REGARDING THE ENTERING INTO A LOAN AGREEMENT FOR THE REFUNDING AND
NEW ISSUE OF

REVENUE BONDS
Notice is hereby given that a public hearing will be conducted by the Board of County Commissioners of
the County of Wood, Ohio, on Thursday, June 25th, 2009, at 10:30 a.m. in the Commissioner’s Offices, One
Courthouse Square, Bowling Green, Ohio 43402. The purpose of the hearing is to consider the proposal of
Piping Industry Training Center Trust Fund (the “Borrower”) that the County of Wood, Ohio (the “Issuer”)
approve permanent financing for the costs of refunding Series 2001 Revenue Bonds, the proceeds of which
were used to finance a training facility, and for a new issue of bonds for the renovation and expansion
of said training facility, which is located at 7560 Caple Blvd., Northwood, Wood County, Ohio. The
existing expanded facility will be owned and operated by the Borrower for the training and education of
apprentices and journey persons in the mechanical construction industry. Such financing is to be
effected by issuance by the Issuer, acting under and pursuant to Section 13 of Article VIII of the Ohio
Constitution and Chapter 165 of the Ohio Revised Code, of a maximum principal amount of $2,900,000
industrial development refunding and revenue bonds (the “Bonds”). THE BONDS SHALL NOT CONSTITUTE A DEBT
OR PLEDGE OF THE FAITH AND CREDIT OF EITHER THE STATE OF OHIO OR ANY OF ITS POLITICAL SUBDIVISIONS
INCLUDING THE ISSUER. Interested persons are invited to attend this public hearing and will be given an
opportunity to express their views, both orally and in writing, concerning the location and nature of
the proposed project to be financed and the proposed issuance of Bonds. This notice is given pursuant to
Section 147 of the Internal Revenue Code of 1986, as amended.

The following applications and/or verified complaints were received, and the following draft, proposed,
or final actions were issued, by the Ohio Environmental Protection Agency (OEPA) last week. “Actions”
include the adoption, modification, or repeal of orders (other than emergency orders); the issuance,
denial, modification or revocation of licenses, permits, leases, variances, or certificates; and the
approval or disapproval of plans and specifications. “Draft Actions” are written statements of the
Director of Environmental Protection’s (Director’s) intent with respect to the issuance, denial, etc. of
