SEALED BIDS for the furnishing of the necessary materials and construction of the
2009 STREETS AND
SEWER IMPROVEMENTS
MILLBURY, OHIO
will be received by the Village of Millbury, Ohio at the office of Engineer — Poggemeyer Design Group,
Inc., 1168 North Main Street, Bowling Green, Ohio 43402 until
11:30 a.m. (Local Time)
Friday, June 19, 2009
and at that time and place will be publicly opened and read aloud.
The scope of work consists of miscellaneous street resurfacing and storm improvement projects in the
Village including two (2) resurfacing projects and three (3) storm projects. Village reserves the right
to award any or all projects.
The contract documents, including plans and specifications, are on file at the office of the Village of
Millbury, Ohio and the Engineer — Poggemeyer Design Group, Inc. (PDG). A set may be obtained from PDG.
A non-refundable deposit in the amount of $35.00 will be required for each set of plans and
specifications, check must be made payable to Poggemeyer Design Group, Inc.
The Engineer for the Project is Poggemeyer Design Group, Inc., 1168 North Main Street, Bowling Green,
Ohio 43402.
All bids must be signed and submitted on the blanks which are bound in the specification booklet. Bid
must state the unit prices in the blanks provided and be enclosed in a sealed envelope marked —
2009 Streets and Sewer Improvements, Millbury, Ohio — and addressed to
the Engineer — Poggemeyer Design Group, Inc., 1168 North Main Street, Bowling Green, Ohio 43402.
The bid guaranty may be of two forms:
1. A Bid Guaranty and Contract Bond using the form in the Contract
Documents. (The amount of the bid does NOT have to appear on this form.)
2. A certified check, cashier’s check or letter of credit in favor of the
Village of Millbury, Ohio, in the amount of 10% of the bid. If the contract is awarded, a Contract Bond
will be required, which is a 100% payment and performance bond.
After the award of the contract let by competitive bid and prior to the time the contract is entered
into, bidders shall submit the affidavit required under the Ohio Revised Code, Section 5719.042 that the
bidder was not charged with any delinquent personal property taxes in Wood County, Ohio.
The successful bidder will be required to pay not less than the minimum wage rates established by the
Department of Industrial Relations of the State of Ohio.
The Village of Millbury, Ohio reserves the right to reject any and all bids and to waive any irregularity
in any bid and to determine the lowest and best bidder.
No bidder may withdraw his bid for a period of 60 days after the scheduled closing time for the receipt
of bids.
The estimate for the cost of construction is $93,000.

The proposed work consists of the rehabilitation of sanitary sewer manholes and 18-inch and 21-inch
sanitary sewer including all necessary appurtenances and accessories as required.
The contract documents, including drawings and specifications, are on file at the office of the City
Engineer, City Administrative Services Building, 304 North Church Street, Bowling Green, Ohio. A set may
be obtained by depositing $50.00. Checks shall
be made payable to the City of Bowling Green, Ohio.

The full amount of the deposit for the first set of documents obtained will be refunded to each bona fide bidder or vendor who returns the
completed documents in good condition within ten (10) days after the opening of bids. The full amount of
the deposit will be returned to each non-bidder who returns the documents in good condition prior to the
bidding date.
Additional copies (in excess of one) of the Contract Documents may be purchased at $50.00 each. No refund will be allowed for the return of the said
additional copies.

All bids shall be signed and submitted on the blanks which are bound in the specifications. Bid blanks
shall not be removed from the specifications. Bids shall state the unit prices on the blanks provided

Each bid must contain the name of every interested person, firm, or corporation and be accompanied by a
bid guaranty to the City of Bowling Green as described in the contract documents.
The bid guaranty may be of two forms:

the forms provided in the specifications. (The amount of the bid does NOT have to appear on this form.)

The Successful Bidder will be required to submit an affidavit to the City Finance Director that he has no
delinquent personal property taxes charged against him on the general tax list of personal property in
Wood County, Ohio, at the time this Bid was submitted.
Bids received after the scheduled bid opening date and time, or not accompanied by a satisfactory bid
guaranty, will neither be read nor considered.
The Successful Bidder and any and all Sub-Contractors must comply with the provisions of the Ohio Revised
Code, Chapter 4115, and will be required to enter into a separate Public Improvement Agreement with the
OWNER prior to the execution of the Contract Documents. A sample copy of said Public Improvement is
included in the Appendix.
The Successful Bidder will not discriminate against any employee or applicant for employment because of
race, religion, color, sex, national origin, disability or age. The Successful Bidder will take
affirmative action to ensure that applicants for employment are considered for employment and that
employees treated during employment, without regard to their race, religion, color, sex, national
origin, disability or age. The Successful Bidder will also incorporate the requirements of this
paragraph in all of its respective contracts for subcontracted work. Further, he/she agrees that they
and any subcontractor shall comply with Ohio Administrative Code 5501-1-02, Appendix B, State Equal
Employment Opportunity Covenant.
The City of Bowling Green, Ohio reserves the right to reject any and all bids and to waive any
irregularity in any bid and to determine the lowest and best bidder.
The City reserves the right to award the base bid and including or substituting any alternate bid items
listed in these bid documents.
No bidder may withdraw his bid for a period of 60 days after the scheduled closing time for the receipt
of bids.

Notice is hereby given that the Board of Township Trustees of Milton Township, Wood County, Ohio, will
receive sealed bids until the 23rd day of June, 2009 at 8:00 PM for the furnishing of all labor,
materials, tools, machinery and equipment necessary for improvement of various township roads, all in
accordance with the estimate as prepared by the Wood County Engineer, to wit:

PUBLIC NOTICE
The following applications and/or verified complaints were received, and the following draft, proposed,
or final actions were issued, by the Ohio Environmental Protection Agency (OEPA) last week. “Actions”
include the adoption, modification, or repeal of orders (other than emergency orders); the issuance,
denial, modification or revocation of licenses, permits, leases, variances, or certificates; and the
approval or disapproval of plans and specifications. “Draft Actions” are written statements of the
Director of Environmental Protection’s (Director’s) intent with respect to the issuance, denial, etc. of
a permit, license, order, etc. Interested persons may submit written comments or request a public
meeting regarding draft actions. Comments or public meeting requests must be submitted within 30 days of
notice of the draft action. “Proposed Actions” are written statements of the Director’s intent with
respect to the issuance, denial, modification, revocation, or renewal of a permit, license, or variance.
Written comments and requests for a public meeting regarding a proposed action may be submitted within
30 days of notice of the proposed action. An adjudication hearing may be held on a proposed action if a
hearing request or objection is received by the OEPA within 30 days of issuance of the proposed action.

action issuing, denying, modifying, revoking, or renewing a permit, license, or variance which is not
preceded by a proposed action, may be appealed to the ERAC by filing an appeal within 30 days of
issuance of the final action. ERAC appeals accompanied by a $70 filing fee which the commission in its
discretion may reduce if by affidavit the appellant demonstrates that payment of the full amount of the
