Bidders may submit requests for consideration of a proposed Substitution for a specified product,
equipment, or service to the Architect/Engineer (”A/E”) no later than 10 days prior to the bid opening.
Additional products, equipment, and services may be accepted as approved Substitutions only by written
Addendum.
From time to time, the State Architect’s Office issues new editions of the “State of Ohio Standard
Requirements for Public Facility Construction” and may issue interim changes. Bidders must submit bids
that comply with the version of the Standard Requirements included in the Contract Documents.
Prevailing wage rates and Equal employment Opportunity requirements are applicable to this Project.
This Project is subject to the State of Ohio’s Encouraging Diversity, Growth, and Equity (”EDGE”)
Business Development Program. A Bidder is required to submit with its Bid and with its Bidder’s
Qualifications form, certain information about the Certified EDGE Business Enterprise(s) participating
on the Project with the Bidder. Refer to subparagraph 6.1.12 of the Instructions to Bidders.
The EDGE Participation is not required for this project.
DOMESTIC STEEL USE REQUIREMENTS AS SPECIFIED IN OHIO REVISED CODE SECTION 153.011 APPLIES
TO THIS PROJECT. COPIES OF OHIO REVISED CODE SECTION 153.011 CAN BE OBTAINED FROM ANY OF THE OFFICES OF
THE OHIO DEPARTMENT OF ADMINISTRATIVE SERVICES.
Bidders are required to be enrolled in and to be in good standing in a Drug-Free Workplace Program
(”DFWP”) approved by the Ohio Bureau of Workers’ Compensation (”OBWC”) prior to submitting a Bid and are
required to provide, on the Bid Form with its Bid, certain information relative to their enrollment in
such a program; and, if awarded a Contract, shall comply with other DFWP criteria described in General
Conditions Paragraph 1.10 – Drug Free Workplace Program Participation.
Bidders entering into a contract greater than $2,000,000 are required to submit their bid information
into escrow. Refer to subparagraph 6.1.10 of the Instructions to Bidders.

NOTICE
In accordance with Section 114.17(b) of the Codified Ordinances of the City of Bowling Green, Ohio, the
Taxicab License Board hereby advertises the Rates of Fare schedule for the proposed taxi service dba:
“Scott’s Taxi” owned and operated by Scott M. Ellinger in a vehicle marked “Scott’s Taxi.” Passengers
picked up within the corporate limits of the City of Bowling Green, will be charged a flat rate of $5.00
per person regardless of the number of passengers for a one-way trip. The number of passengers is
limited by the legal capacity of the vehicle in operation. Seniors age 55 and over with identification
may request $1.00 off discount for one way trip within the corporate limits. BGSU Students with
identification may request $1.00 off discount for one-way trips within the corporate limits.
Scott’s Taxi will be available by appointment only during posted hours of operation for trips outside the
corporate limits of Bowling Green not to exceed fifty (50) miles one way. The rate of fare for trips
with a destination outside of the corporate limits is $1.50 per mile.
The proposed hours and days of operation for Scott’s Taxi Service are Monday through Saturday 3:00 p.m.
to 3:00 a.m.; Sunday 7:00 a.m. to 7:00 p.m. and Holidays 7:00 a.m. to 7:00 p.m.
The proposed foregoing operation schedule is on file with the Taxicab Board of Review, 304 North Church,
Bowling Green, and will become effective automatically two weeks from June 1, 2009, unless objections
are filed by residents of the City prior to June 15, 2009. Persons wishing to object to the proposed
rates should file their objections in writing to the Taxicab Board of Review. Objections, if filed, will
be reviewed during a hearing of the Board.

The Lake Township Trustees will accept sealed bids at their office, 27975 Cummings Road, Millbury, Ohio,
43447, until July 7, 2009 at 5:00 pm. Said bids will be opened immediately thereafter for the following
Road Resurfacing projects.
Keller Road (T-311) from East Broadway to Luckey Road, Length: approx. 1 mile
Libbey Road (T-308) from Lemoyne Road east to concrete. Length: approx. .4 miles
Specifications may be obtained at the office 8:30 to 4:30 p.m. Monday through Friday or by calling

A bid bond or cashier’s check in the amount of five percent (5%) must accompany each bid. A one hundred
percent (100%) performance bond shall be issued by the successful bidder.

All bidders shall submit a statement that they are not charged at the time the bid was submitted with any
delinquent personal taxes as per O.R.C. Section 5719.42.
