Jacquie R. Fields, 75, of Risingsun, died Jan. 26, 2012, at Bridge Hospice Care Center in Bowling Green.
There were no services held and interment was private.
Memorial contributions are requested to CASA of Bowling Green.
Mann-Hare-Hoening Funeral Home, Fostoria, has handled arrangements.
