|Family Services of NWO earns its Health Home certification|

Family Service of Northwest Ohio has received its Health Home certification from Ohio Department of Mental Health to provide integrated mental and physical health care to clients.
A product of the recent health care reform initiative, the Health Home is responsible for the coordination of physical and behavioral health care for individuals with serious and persistent mental health conditions.
Family Service of Northwest Ohio is located in Toledo, but it includes a service location, the Family Service Counseling Center of Wood County, at 1084 S. Main St. in Bowling Green.
"This integrated approach addresses the critical link between physical and mental health," emphasized James E. Jones, president & CEO of Family Service of Northwest Ohio. "This king size safety blanket of care will serve the population of Northwest Ohio more effectively and thoroughly than the former one dimensional treatment could ever provide."
The Health Home includes a multi-disciplinary team: a health home team leader, care manager and a qualified health home specialist, at a minimum. Other members of the team may include a M.D., LPN, peer support specialist, and/or dream manager. This team provides all the Health Home components: comprehensive care management, care coordination, health promotion, comprehensive transitional care, individual and family support services, and referral service to community and other social services.
Programs and services are funded in part by Lucas County Board of Mental Health & Recovery Services, Wood County ADAMHS Board, Four County ADAMHS Board, the Area Office on Aging, Ohio Department of Jobs & Family Services, United Way of Greater Toledo, United Way of Fulton, Defiance, Henry and Williams counties,
