Palicki Address Unknown You are notified that on the 23rd day of July, 2013, a Petition for the Adoption of Cameron Blake Pollard, who was born on the 3rd day of December, 2000; was filed in the Probate Court, Wood County, Ohio and that the hearing on said petition will be held before the Judge in said Court on the 11th day of December, 2013, at 9:00 a.m. It is alleged in said petition that you as natural parent of said child have failed without justifiable cause to provide more than de minimis contact with the minor or to provide for the maintenance and support of the minor, as required by law or judicial decree for a period of at least one year immediately preceding the filing of the Adoption Petition.
