001 LEGALINVITATION TO BID SEALED BIDS for the furnishing of the
necessary materials and construction of the ROSSFORD PHASE II
RIVER ROAD 12” TRANSMISSION LINE NORTHWESTERN
WATER AND SEWER DISTRICT will be received by the
Northwestern Water and Sewer District, 12560 Middleton Pike, Bowling Green, Ohio 43402 until
10:00 a.m. (Local Time) Thursday,
June 13, 2013 and at that time and place will be publicly opened and read
aloud. Thescope of work consists of the installation of approximately 19,850lineal feet of 12” PVC
waterline, (1) pre-fabricated master meter pit,8,000 lineal feet of 1” service lines with 209 meter pits,
hydrants,gate valves, and general restoration, including asphalt, curb, andsidewalks. The contract
documents, including plans and specifications,are on file at the office of the Northwestern Water and Sewer
Districtand the Engineer — Poggemeyer Design Group, Inc. (PDG). A set may beobtained from Becker
Impressions, 4646 Angola Road, Toledo, Ohio 43614,Telephone 419-385-5303,www.pdg planroom.com.
Anon-refundable deposit in the amount of $100 will be required for eachset of plans and specifications;
check must be made payable to BeckerImpressions. The Engineer for the Project is Poggemeyer Design
Group,Inc., 1168 North Main Street, Bowling Green, Ohio 43402. All bids mustbe signed and submitted on the
blanks which are bound in this booklet.Bids must state the unit prices in the blanks provided and be
enclosedin a sealed envelope marked — Rossford Phase II River Road 12” Transmission

Bowling Green, Ohio 43402.The bid guaranty may be oftwo forms:1. A Bid Guaranty
and Contract Bond using the form in the Contract Documents. (The amount of the bid does NOT have to appear
on this form.)2. Acertified check, cashier’s check or letter of credit in favor
of theNorthwestern Water and Sewer District, in the amount of 10% of the bid.If the contract is awarded, a
Contract Bond will be required, which is a100% payment and performance bond.This procurement is subject to
theEPA policy of encouraging the participation of small businesses in ruralareas (SBRAs).After the award of
the contract let by competitivebid and prior to the time the contract is entered into, bidders shallsubmit
the affidavit required under the Ohio Revised Code, Section5719.042 that the bidder was not charged with any
delinquent personalproperty taxes in Wood County, Ohio.The successful bidder will berequired to pay not less
than the highest applicable minimum wage ratesas established by the federal Davis-Bacon Wage Determinations
issued bythe U.S. Department of Labor.DOMESTIC STEEL USE REQUIREMENTS
