NOTICE OF SECOND PUBLIC HEARING FY2014

Grant
Residential Public Infrastructure Grant
Program
The Village of Bloomdale intends to apply to the Ohio Department of
Development (ODOD) for funding under the Community Development Block
Grant (CDBG) Residential Public Infrastructure Program, USDA Rural
Development, and possibly the County’s Revolving Loan Fund (RLF)
Programs, federal programs administered by the State of Ohio.
The
Village is eligible to apply for funding under the Residential Public
Infrastructure Program and if approved, to allocate matching RLF funds
toward any water/sewer projects, if funds are available. The Village is
applying for an estimated
infrastructure/ administration grant of
approximately $500,000 of FY-14 CDBG Residential Public Infrastructure
Program funding for assistance to construct a new waterline. The first
of two required public hearings was held April 8, 2014 to inform
citizens about the CDBG program, eligible activities, and other
requirements.
The Village of Bloomdale project consists of the
construction of a water transmission main, which will tie into the
existing 12” waterline on Jerry City Road at the intersection of
Bloomdale. The waterline will travel south on Bloomdale Road
approximately 5.55 miles and connect into the existing water
distribution system located within the Village of Bloomdale. The total
length of pipe to be installed is approximately 29,347 LF, which
included 5,280 LF of 12” and 24,067 LF of 8” pipe. Most of the waterline
will be installed inside acquired permanent easements along Bloomdale
Road or in the public road right-of-way.
Other appurtenances include
gate valves & boxes, tracer wire test stations, rock excavation,
fire hydrant assemblies, package booster pump station, 3 pumps, VFD
units; blower; chlorinator; stabilization, SCADA controls, electrical,
site work, standby power, replacement of mixer in existing elevated
tank, asphalt & concrete drive replacement, restoration,
preconstruction video, legal, engineering, and testing.
Once the new
line is constructed, the Village’s existing water plant will be
abandoned and the Village will be connected to the City of Bowling
Green’s water supply and will be operated/serviced by the Northwestern
Water and Sewer District.
A second public hearing will be held by
the Village on April 22, 2014 at 6:30 p.m. in the Village Council
Chambers, 206 Vine Street, Bloomdale, Ohio 44817, to give citizens an
opportunity to review and comment on the Village’s proposed projects
before the grant application is submitted to the Office of Community
Development.
Citizens are encouraged to attend this public hearing
to express their views concerning the applications. Should any
participant require auxiliary aids due to disability or non-English
languages, please contact this office at least one week prior to the
hearing date to ensure needs will be accommodated. Citizens wishing to
submit written comments prior to the hearing may direct them to the

The successful bidder will be required to pay not less
than the minimum wage rates established by the Department of Industrial
Relations of the State of Ohio in accordance with all provisions of the
Prevailing Wage Act of the State of Ohio, ORC Sections 4115.03 and
4115.16 and related requirements.
Bids received after the scheduled
bid opening date and time, or not accompanied by a satisfactory bid bond
or check, will neither be read nor considered.
The Board of County
Commissioners may reject any or all of the bids.
The Board of County
Commissioners reserves the right to waive any irregularities in the form

Bids will be received by the Wood County Commissioners at their
offices located at One Courthouse Square, Bowling Green, Ohio until 3:00
p.m. on Friday, April 25th at 3:00 p.m. and at that time and
place will be publicly opened and read aloud.

The estimate of
probable cost for this project is $89,000.00.

Bids must state the

addressed to the Wood County Commissioners, One Courthouse Square, 5th
floor, Bowling Green, Ohio 43402.
The bid guaranty may be of two
forms:
1. A Bid Guaranty and Contract Bond using the form in the
Contract Documents.
(The amount of the bid does NOT have to appear on
this form.)
2. A certified check, cashier’s check or letter of
credit in favor of the Wood County Commissioners, in the amount of ten
percent (10%) of the bid.
If the contract is awarded a Contract Bond
will be required, which is a one hundred percent (100%) payment and
performance bond.
Bidders shall submit with their bid the affidavit
required under the Ohio Revised Code, Section 5719.042 that the bidder
was not charged with any delinquent personal property taxes in Wood
County, Ohio.
The Contractor declares that its principal officers,
directors, shareholders and/or partners are current with any
court-ordered child support payments pursuant to the Board of County
Commissioners’ Resolution No. 92-2041.
All waste materials generated
from this project must be taken to the Wood County Landfill for
disposal.
The successful bidder will be required to pay not less
than the minimum wage rates established by the Department of Industrial
Relations of the State of Ohio in accordance with all provisions of the
Prevailing Wage Act of the State of Ohio, ORC Sections 4115.03 and
4115.16 and related requirements.
Bids received after the scheduled
bid opening date and time, or not accompanied by a satisfactory bid bond
or check, will neither be read nor considered.
The Board of County
Commissioners may reject any or all of the bids.
The Board of County
Commissioners reserves the right to waive any irregularities in the form
of the bid that do not affect or destroy competitive bidding.
In no
case will an award be made until all necessary investigations are made
as to the qualifications of the bidder to whom it is proposed to award
the contract
No bidder may withdraw his bid for a period of sixty
(60) days after the scheduled closing time for the receipt of bids.
