As part of public health disaster response planning efforts, the Wood County Health District is inviting its residents to participate in a short Disaster Preparedness Survey.  
The purpose of this survey is to provide a forum for public input on the level of disaster preparedness in the community and the potential local area risks. The results from this survey will be used to help Wood County agencies better plan for disasters, as well as well as communicate with residents on mitigation steps to reduce the risk of disaster-related loss.
Local residents are being asked to fill out the survey to test their readiness for potential disasters.
The survey will take approximately 10 minutes to complete.  Those finishing the survey have the option to enter into a drawing for an emergency kit.
To access the survey, go to https://www.surveymonkey.com/s/WoodCountyDisasterPrepSurvey or you may access the survey from our website at www.co.wood.oh.us/healthdepartment.  
The survey will close on Dec. 14.
